Sometimes it is just a whole lot of poo from a growth spurt or something (I think?) and nothing can hold it... but the target ones have worked the best for us. For us it's all about position -- if he's not flat and there's pressure on his behind, it goes up the back. When DS was a newborn, I didn't realize there was elastic on the inside of the leg part and once I started pulling that part out, his blowouts stopped.
