@MrsDragon: ah ok. I ask because I was pretty sure/thought I had thrush as well when LO was small and we were dealing with latch issues. I may have (I took diflucan) but what I think it really was was vasospasms, caused from LO's poor latch and his clamping. Taking magnesium daily helped that sooo much. Just something I thought I would mention in case it helps you! :)

@MrsDragon: you could get her a probiotic as a precautionary measure and it might be a good idea to sterilise any paci's, shields or bottles you're using.

Thrush sucks so hopefully you get on top of it soon!

@Kemma: She isn't but I'm going to ask if she should on Monday especially with the open wound in her mouth.
@hilsy85: The pain is more like a scratchy sunburned/chafed kind of feeling. She's definitely still sucking really hard and compressing tight even with a deep latch. Sometimes my nipples look a bit lighter but never the areola.

@MrsDragon: looks like there is definitely some improvement! It will also take time for her to learn her tongue's full range of motion, so don't get discouraged if she's not keeping her tongue out while latching on yet--she will! Just keep doing the exercises! She is ADORABLE!

Also, when you say that you're having pain--is it like a throbbing pain that comes and goes after you nurse? And do you ever notice that your nipples/areola are slightly lighter after she unlatches?

I also used APNO and loved that stuff!

@MrsDragon: Is your LO being treated for oral thrush? One of the LC's I saw wondered if A's latch was off because thrush was making her tongue itchy!

@Kemma: They thought I had it back the first time I went to the LC at the start of the month, but just a mild case so they gave me APNO and any scratchy feeling went away pretty quick, but my nipples and areola stayed red. But that also coincided with me REALLY noticing her compressing and sucking hard, so I chalked it up to latch pain. Then a few days ago I went to see the public health nurses, mentioned the pain had gotten even worse since the day before that, and they took a guess that I had thrush. Especially because I started getting the stabbing needle pain. :( Right now it's still a scratchy/burning pain but the redness on the areola seems to be subsiding a bit!

On the downside -- found out today that her weight is off the curve it was on. She lost a few ounces this week and didn't gain enough overall this month. I'm hoping it was just due to one bad, engorged, her getting frustrated and falling asleep weekend with the shield.

Fwiw, 7-8 weeks was a real turning point for us, we had both ties clipped and LO had a small growth spurt!

We're on day one post-clip and things DO seem to be improving a bit but I'm not sure if it's the clip or because I'm being treated for thrush, hah. Her mouth felt less tight right away and I am noticing less compression especially on the 'good' side so I think the clip did help. Still have pain and compression, but I think it's getting better.

We're doing suck training but she still isn't bringing her tongue over her gums to latch. She'll stick it out and play with it until I bring her to my boob and then it's back behind her gums again. Hopefully we can snap her out of it.

I attached a before and after! The doctor said she had good mobility to begin with and really, it has only given her a tiny bit more tongue to stick out, but if that's what we needed, that's what we needed.

@MrsDragon: my DD couldn't take Soothies brand pacis until she was clipped (actually lasered) either, then they were her favorites. It's really so very worth it, and it will benefit your LO in many ways too, from allowing adequate nutrition, preventing swallowing air and having painful gas, colic and vomiting, allowing for better sleep, preventing oral aversions, food pickiness, and speech issues later on, etc. Another important part, even though it may sound silly, is to have 1-2 sessions of "bodywork" once the ties have been revised. Bodywork for infants is light touch adjustments, usually craniosachral therapy, that helps them loosen up after tightening and making overcompensations for a jaw that was restricted due to ties. My DH was an extreme skeptic, but we took our DD twice after her ULT and PTT were revised, and he saw it help our DD so much that he even went a couple of times for his own leg issue! She was a much more relaxed, happy and different baby after we had the procedure and bodywork done. You're doing the right thing.

@MrsDragon: don't feel that way! Honestly, the sooner the better. My LC said prolonged latch/transfer problems can lead to decreased supply so if we'd had to wait, she wanted me to pump after every nursing session to make sure that didn't happen. And it takes a little while for LOs to re-learn how to nurse properly, so there's really no upside to waiting. It seems crazy that 1-2mm of flesh can make THAT much difference, but it really can.

@PurplePeony: M is pretty much incapable of taking a soother! We've had the most luck with Soothies but I have to hold it in her mouth. She can suck on it on her own for a minute or so if I have held it in long enough but she slowly pushes it out, much like she does to my nipple.

I'm suuuuuper nervous about tomorrow -- I feel like I'm somehow doing something wrong by pushing the schedule up. Argh.

@MrsDragon: I think you made a good decision. It's been almost two weeks since we had my DD's clipped and I'm seeing a huge difference in her ability to stay latched, transfer milk, and she spits up way less than before. I initially felt guilty because I felt like I was putting her through pain for my own benefit, but we're both so much happier now that I'm really glad we did it. An added bonus is that she can now hold a pacifier in her mouth rather than having it immediately pop out, which has been great for those times when she wants to suck but not eat. Incidentally, the doctor who clipped DD's tie only recommends pacis with straight nipples, like Soothies, because she said the other shapes don't promote good sucking habits -- just thought I'd throw that out there.

Good luck with the clip, I found it to be far less traumatic than I expected. It took about 30 seconds and then she nursed a bit and was totally fine. Just be diligent about the aftercare!

D2 had a posterior tongue tie that was clipped twice (because she had regrowth after the first) AND she had a lip-tie (the worst her ped had ever seen...Ugh!). My LC was an advocate for clipping if it would help me breastfeed better.

@MrsDragon: like you said, doing the clip WILL NOT make anything worse at all, and can ONLY improve things. Your LC is not the one nursing, so don't let her pressure you if you feel like clipping is the way to go. I was also told that my LO's case was borderline based on the Hazelbaker scale, but it definitely helped, ALONG WITH doing the tongue stretching exercises.

Definitely advocate for yourself!! It sounds like you are informed about posterior tongue ties, which is great!

@PurplePeony: @MrsRcCar: I'm not sure that the suck training is to address the possibility of a tongue tie, specifically, because the LC isn't convinced she has one. On the Hazelbaker assessment she did, M came up as 'consider frenectomy if management is unsuccessful' or something like that. She scored an 11 on function which is only ok if appearance is above 10, and she got a 9 on appearance. So she's really borderline but the LC said her frenulum is stretchy.

So, she's focusing more on the tongue dysfunction that she seems to have, and her killer gag reflex. To 'train' her, we're supposed to use our pinky and have her suck on it, with it in the soft palate where my nipple is supposed to go. That's when she usually gags and tries to push it out with her tongue, so I guess we're sort of desensitizing her. The other aspect is to rub her lower gums and upper gums, and around her mouth. I'm not sure how it works but it's supposed to loosen things up. The LC also ordered a book on suck problems because she thinks M is disorganized with her sucking (and I agree -- I just think there might be something more to it).

A lot of the literature I've read indicates that if there's a lot of maternal pain, a clip should be considered even if things don't exactly add up to an obvious tie. The LC said she just doesn't like to rush into it in case parents clip and see no result, because then you've intervened needlessly. I totally get that, but I feel like we've tried EVERYTHING and so many of our symptoms point to a PTT. And she was pointing out that she can bring her tongue past her gums, that she doesn't have the heart-shaped tip, and I realize that, but that's not the type of tie I think she has.

I'm going to do the training, and we also picked up a hospital pump loaner to try and get me a weekend or a weekend day off of nursing. My nipples are SO sore. We have seen some improvement since last week -- she comes at me with an open mouth more often, she brings her tongue out over her gum more often, and on the left side, she looks like she has a solid latch more often (but it still hurts). But I have a feeling we will be going back in to talk about a clip.

I'm wavering, though. My gut was telling me that it had to be a tie and I wanted a clip. But the LC seems so sure that it isn't neccesarily the problem. I don't think it'd hurt to do it, but I don't know. Argh.

@MrsDragon: If you have more information about the suck training I would be interested to read it! We found it 100% helpful to have N's Lip and tongue tie clipped. We had to look to find an ENT that took care of tongue and lip ties, but we found one about 1.5 hours from them. I love his ENT she is the nicest lady! I will set up with her again if LO#2 has the same issue. (Which is likely because it runs in our family like crazy)

Sounds like DS and looks like it from the picture. DS had an upper lip tie and a posterior tongue tie. We found a dentist an hour away who specialized in tongue ties. He was able to evaluate and treat (via laser) within the same visit. I was seen by 3 LC none of which thought anything was wrong, despite my pain and lipstick flat nipples after DS nursed. Definitely have it checked out.

@MrsDragon: Be sure to have your lo checked for a lip tie if they find the tongue tie. My ENT said it is common from the two to be hand in hand. Good luck with everything! There was no denying N's lip and tongue ties.

Sounds just like my DD, and we also started to have trouble around 4 weeks. I thought she was overwhelmed by my oversupply/forceful letdown, but once I could tell she was dealing better with that but the pain/latch issues/clicking/spilling milk didn't improve at all, I went back to the LC for evaluation. My DD could also stick her tongue out past her gums, but had a posterior tongue tie nonetheless. The LC pulled strings for us to get in and have it clipped that day because she was worried about my supply tanking if the problems continued. It's been just over a week since we had it clipped and things are MUCH better now. It takes awhile for babies to re-learn how to nurse after the tie is clipped, so don't be alarmed if it's not a magic fix -- but I'm so glad we had it done because DD is much happier (less air intake so not as gassy) and more efficient at getting milk out. Good luck getting your LO evaluated and hopefully resolving the problem!
