&#60;p&#62;I would say that you'll be okay with Tums early in the night.  (If you are needing more than one or two, you might need to look into something longer acting anyway!)  &#60;/p&#62;
&#60;p&#62;The reason why they do this is because if they have to put a tube in your throat to help you breath (i.e. all the way under), then you cannot protect your airway anymore and can get stomach contents in your lungs.  But...you're having spinal anesthesia most likely.  Which means you'll be awake.  Which means you can protect your own airway.  &#60;/p&#62;
&#60;p&#62;Short answer, is that it is all a liability thing.  Easier to tell everyone &#34;nothing to eat after midnight&#34; in the off chance that you require emergency intubation.  Because of that, I would always let someone know if you have anything more.  Some anesthesiologists will flip out if the patient chews gum the morning of surgery, even if they spit it out.

<description>&#60;p&#62;I'm sure you shouldn't, but my water broke unexpectedly and I needed a c-section that night.  I had eaten a delicious dinner of turkey three bean chili and corn bread (I'll never forget that because I was terrified I would see it again, I didn't!)  for dinner not three hours before I had my c-section.  So, they can do it!  It's not like they waited 12 hours for mine :)
