LO is 4 months. When she wakes up I feed her. Not instantly, I wait to see if she is going to fall back to sleep, but usually she is turning her head to the side and rooting and then she'll start crying if I don't get the breast to her fast enough.

E is 12 weeks old now and usually only eats once during the night (except for recently due to a growth spurt). He wakes up several times throughout then night, but I go in and give him his pacifier and put my hand on his chest and he usually goes right back to sleep. If I put his pacifier in and he's fussing again within a half hour I assume he's hungry and feed him.
We started this because I was going in multiple times a night to feed him, but he'd only eat for a few minutes and then be asleep. I didn't think he was really waking up because he was hungry and just needed soothing.

My general rule is that I waited a few minutes to see if he'd just fall back to sleep and if he didn't I popped a paci in his mouth. If he woke up a third time, I fed him.

I would wait 5-10 minutes to see if they go back to sleep. Then I would try the pacifier. For a couple of weeks my lo would take the pacifier after waking up after 6-7 hours then fall asleep for another 2-3 hours. He was going 9-10 hours without a feed at 3-3.5 months but not necessarily sleeping that long.

Eta: when he woke up the 2nd time I would nurse him an he would go back to sleep for a total of 12-13 hours in his crib.

DS is almost 5 months, and we wait up to five minutes when he wakes up in the middle of the night before responding. He's a noisy sleeper, so we wait until he's actually crying (and we know he's awake) before getting him. Then, depending on what time it is (if it's at his regular feeding time) we either feed him or try to put him back to sleep without a bottle. He's pretty regular in his nightwakings, so if he wakes up outside of his regular feeding time, I can usually put him right back to sleep.

These past couple weeks as LO has been having a sleep regression, I decided to try and pacify her without picking her up and giving her the boob... to no avail. At this point on her good nights when she's only up once, I think she still actually needs the feed.

I think around 6 months we started to not always feed her when she woke up but she really around that time was just getting up once or twice a night and I kinda figured she was hungry.

Not sure... depends on if you think she is hungry or needs to eat. I do that too because it's easy.

I haven't night weaned DS yet because he was always small for his age. I didn't feel comfortable. But he will be 6 months next week and I will start him on 3 consistent solid meals a day, and I think that will hopefully eliminate the need for a night feeding.

Also my midwife DID tell me that once they reach 14 lbs you can night wean them.

When your LO wakes up in the night and starts to whine, do you instantly pick them up and go to feed them? Or do you try and shush them back to sleep? Pop a pacifier in?

My LO doesn't wake up very often in the night, usually only once or twice, but when she does I just pick her up and pop her on the boob. I assume she is hungry since it has been 5-7 hours since the last time she ate. But I recently realized that maybe I shouldn't be doing this?
