So, this is officially an ectopic pregnancy. Received methotrexate in the ER about an hour ago. Once we get through this my OB is going to refer me to an RE for a HyCoSy, and possibly other testing to see what our options are...

@yellowbeach: Thanks, and it turns out they were probably right about the ectopic, I still feel just as pregnant 24 hrs post EUE, but we never would have known if I hadn’t pushed for the EUE which prompted the doctor to ask for the level that showed HCG rising again. It stinks that another ectopic pregnancy is the answer, but at least we know and can be on the same page. What I don’t know now though is what this means for our future. I understand probability and know that just because we’ve had a two straight ectopics it doesn’t mean we’re going to have a third., but it’s so scary to try again without any real knowledge of what our risk is, because ectopic pregnancy is so rare and there aren’t any studies that my doctor’s can cite about outcomes after more than one. They just push to IVF if I’m nervous, which as you know is a whole other can of worms.

@karenbme: I’m so sorry you are going through this, again. It’s totally reasonable and makes perfect sense that you’d want more answers about why this is happening and how/if you can prevent it in the future. Not just for family planning, but to avoid another life threatening condition like ectopic.

I’m sorry your doctors aren’t listening or aren’t hearing you. Only thing I can think of is that in the world of OB and emergency medicine, ectopic is a super red light condition that’s drilled in to people to not take lightly or mess around with. Maybe they are just totally focused on the current potential for ectopic and until then, can’t bring themselves to focus on anything else. That’s my guess. So sorry.

So they had me do more blood work yesterday and it came back higher than the day before, but only by 5 units. My doctor is more confident than not that it’s a repeat ectopic, apparently they never see levels that change direction in interuterine pregnancies. I go in 2 hours for an EUE to definitively rule out IUP, and I keep going back and forth on whether this is the right thing to do. Really I don’t have any other options, but I wish we could wait until we would expect to be able to see something, so that at least I knew there wasn’t a chance. I’m just so confused, I’ve had no bleeding at all and no pain. I feel like I’m never going to trust my body to do this again. And on top of everything methotrexate is the only option they’re giving me for management when the pathology comes back because I’m 20 points above the HCG threshold for expectant management and they won’t do exploratory surgery even though this would be my second ectopic in 5 months and they admit there’s clearly something wrong with my tubes.

@karenbme: that's so hard since there isn't a clear pattern. But I know exactly how you feel about looking for answers. I told the RE that basically we want to know if there's some clear problem we can fix or whether it's going to continue to be a crapshoot where we have to decide how many miscarriages is too many. It's just the worst.

@Sams Mom: Thanks, I had an appointment with another doctor at the same practice on Friday that I think I’m going to keep. Partly to see what she says and partly because DH will get a chance to ask questions, too.

@Chuckles: Thanks for the perspective, because one of my losses was ectopic and my first mc was at 9 weeks and this loss will be ~6 weeks depending on when I start bleeding the main doctor I’ve been seeing doesn’t think there’s enough of a pattern to warrant testing. I take issue with the chemical pregnancy label though because it’s based on my hormone level not how far along I am, and they only know the hormone level because of the testing related to my ectopic, in 98% of cases this would be a termed a miscarriage at 6 weeks. I just want to know that either there’s nothing they can find, or there’s something we can fix, or we might as well not put ourselves through this again, because there’s something we can’t fix. You know?

@Kaohinani: Thanks, I asked my main doctor three times on the phone today for more testing and the only thing she offered was a referral for for a test to make sure my tubes were open and/or IVF, which was so frustrating. IVF isn’t going to stop my babies from dying. I’ll see what the other doctor from the practice says on Friday, but I might take the IVF referral and hijack it to ask the RE for more testing.

@karenbme: I am absolutely gutted at this very moment as I had the highest hopes that your pregnancy would have been successful. I am so absolutely sorry for all that you are experiencing at this time and I can only offer my heartfelt condolences. I know my opinion offers no comfort but, honestly, I would ask for further testing. I only state this as my godson was a similar story but, somehow, is a living, breathing 2-year-old today. All of us (hellobees) support any decision you make. Anyway, just know my own personal thoughts are with you through this difficult time. 💜 *huge hug*

I'm so sorry you are going through this again. I'm surprised they are saying no to testing. Just for perspective, I just had my 3rd loss this year and, even though 2 were pretty early, I just had my first RE appointment yesterday and my insurance is covering lots of testing. It couldn't hurt to get a second opinion if that's the route you want to take.

@Shantuck: Thanks. ❤️

So the results from my blood work this morning came back as a 25% decrease compared to 2 days ago. We may never know where it implanted, and the doctor is counting it as a chemical pregnancy even though I’m almost 6 weeks and we only know it’s failing because of the extra blood work. Chemical pregnancy means they won’t test me for anything linked to repeat miscarriage. I don’t know if I should get a second opinion or what. It just feels hopeless right now.

@karenbme: Just wanted to say that I'm sorry you are going through this again. I had a loss (not ectopic) that also had low rising hcg levels so that doesn't always signify an ectopic pregnancy. They were definitely worried about that, though, with my levels. I can totally get why you'd be worried given your history. Sending good thoughts your way that you don't have to go through that again.

@Kaohinani: Thank you, lady. You are literally the sweetest. I’ve been telling myself those things over and over, and trying to remember that if not for my history I would have no idea what my levels are right now or that anything might be wrong. Right now I have to hold out hope that I have a fighter. One of the ways I work against anxiety, though, is to make plans for every eventuality, so for me “this baby is still growing” and “I might be in surgery getting an ep removed on Friday” and “another miscarriage would be preferable to another ectopic” are all necessary and useful thoughts to hold in order to prepare myself for whatever the future may bring. Really, I was looking for info on what my future looks like if it is another ectopic pregnancy, because about 35% of the time they’re asymptomatic. Also the doctor mentioning methotrexate on the phone really didn’t help yesterday, because that was the worst hell I’ve ever been through and I even though I know last time I needed 3x the dose it usually requires and probably wouldn’t need that much a second time, I really don’t want to do it again.

@lilyofthewest: Thanks, I’ve seen the data on discriminatory zone in us as well, but two different doctors from my OB practice have told me that they want to see me that day, so I trust they know what they’re looking for. Plus there’s a whole algorithm we’ll go through if they don’t see anything Friday that starts with more blood work and checking again. I’ve already hit my deductible for the year and the ep in the spring was ~1 cm and very visible in my tube when my HCG was ~1000, so if they think they might see something I’m gonna go for it. I know it might be more waiting and stress but if I get answers a few days earlier it’s worth it.

@karenbme: I am so, absolutely, sorry that you are feeling so much uncertainty at this time and that you feel so very guarded about this pregnancy that you can not enjoy the experience. :heart: *hugs* Keep in mind though, until you have a definitive answer, this pregnancy is not the same as your ectopic loss. Your numbers may be slow at the moment but many healthy babies had a "slow start" only to thrive... one being my godson. At this time, you don't know the attachment point (uterine vs a tubal attachment) and have yet to demonstrate the same symptoms as your last ectopic loss. Please, live in the moment and stay hopeful that this will be a tiny survivor. (I know, easier said than done, eh? You KNOW my record with anxiety so, please know, I'm telling you what I'd wish to hear in the same situation. 🧡) Tests will come soon enough and you will know for certain, but, until then, you have tons of support here. Please, keep us updated. My thoughts are with you.

I had four losses between LO and my current pregnancy. Loss #3 was an ectopic. Trying to figure out how and when to manage a possible ectopic is really tricky. You're very unlikely to be able to see anything with ultrasound until beta hCG is above 1000. It isn't unusual to not see anything until the beta is somewhere between 1500 and 2000. Surgical management isn't a great option without a known, visualized ectopic. But, on the other hand, ruptures can happen when betas are still in the 100s. Hoping it is "just" a failing intrauterine pregnancy.

@mrskansas: Thanks. Nobody should have to go through this.

Your ectopic story sounds a lot like mine. Same not really a period, same spotting, but my doctor started monitoring for ectopic right away, and I was diagnosed at ~6 weeks. I'm hoping since I've had none of the symptoms this time it's an IUP, at least that way I keep both my tubes, we don't lose time, and I can beg my doctor for a repeat pregnancy loss work up.
