As my midwife described it, the well-woman exam checks a number of things and should be done yearly. The PAP is often done at that appt, but is actually recommended every 3 years now. So they expect to see you yearly but won't necessarily do a pap every time.

Because I had just suffered a loss, I didn't want anyone going near my cervix. We ended up nixing the pap (I had one 2 yrs prior) and putting off the rest of the internal exam stuff until my 11 week appt. But she said I couldn't skip the internal exam entirely. The state required a couple of things from it, like a gonorrhea a swab.

The more replies, the more I am still unsure. I doubt the actual doctor will be much help when I see him, but it's probably good to hear from him about what he can/ cannot do versus the appointment scheduler.

They're going to want to do a pap exam at your first appointment, so just up to you if you want to get it out of the way or not. Personally, I opted out of it because I'd had a pap done very recently, but it sounds like that's not the case for you.

I believe the new guideline is every 3 years. I would skip, in fact I DID skip myself in pregnancy.

I had my annual in December & got pregnant that same cycle.

My OB did another pap at my 8w appointment, so that means I had one in December and one in February!

My last midwife said she doesn't bother with smears during or immediately after pregnancy as the cells are often abnormal anyway (due to the pregnancy I guess), interesting that the guidelines are different in the US!

@Mrs. Lemon-Lime: I didn't actually schedule mine to be combined, I just scheduled it as my first pregnancy appt/dating u/s. When I met with the nurse/OB they asked me a ton of background questions including when was your last pap. When I told them it had been over a year they said they'd go ahead and do one that day.

Like others, the well woman exam is part of the first pregnancy appointment (following the initial confirmation appointment). I go for mine next week at 11 weeks. It was the same last time and I did not have any spotting following the Pap.
