@PurplePumps: With my IUI cycles I take clomid CD3-7 go in for a folllicle check CD12 depending on size I trigger 12 hrs before my scheduled IUI my RE does one 12 hrs after trigger and one again 36hrs after trigger. I take Noveral when I trigger, I do know that is a BIG surge it will make sure you Ovulate from both sides I believe and it also helps to time IUI as much as possible so sperm can meet the egg.

I was also given progestrone (Crinone) just as an incase it can't hurt you! But can get pricey...I think mine was $16 per applicator per day....

Best of luck to you!!!

@PurplePumps: During my IUI cycles, they had me on the same combo: Clomid, Ovidrel, progesterone. There was a time or two when my LH was already surging, and the timing of the surge worked out nicely with the size of the follicle, so they told me I didn't need to take the Ovidrel because they knew I was going to O on my own in the window that they wanted. Good luck!

@PurplePumps: I don't think you would trigger if they confirmed you already ovulated, but I don't know that for a fact!

I guess it's also possible that between the time you went in for monitoring and the time they told you to trigger, you could ovulate. Like if you went in on a Thursday and they told you to trigger Saturday, you could ovulate on Friday and not know it.

@PurplePumps: Okay, so here's how my cycles went:

Cycle 1: Clomid + Ovidrel. I didn't respond well to Clomid, I started at 50mg and in the same cycle did 100, then 150. I eventually ovulated and had a CP.

Cycle 2: Clomid. I started at 150mg of Clomid, then 200mg, made no progress and cycle was cancelled because 200mg is the max my RE will do.

Cycle 3: Femara + Ovidrel. Started Femara at 2.5mg, then 5, then 7. Eventually ovulated, might've had a CP, but was gone by beta day.

Cycle 4: Femara 7mg + Gonal-F 75iu + Ovidrel: Success! Sticky baby thats 10 weeks old :).

The important thing to know about Ovidrel is that it causes false positives on pregnancy tests. So you have to decide to either wait it out until at least 14 DPO to test, or you need to test continuously so you can see it fade out or get stronger. I started testing from about 5DPO, and usually by 8 or 9 the trigger would be gone. Cycle 3 it stuck around really faintly until 12DPO, so it's possible I had a CP then too. On the cycle I got pregnant at 8 and 9 DPO I was still getting positives, but outside the 10 minute testing window. At 10DPO I got a positive within the timeframe.

@Happygal: So if you happen to ovulate before the trigger, then you wouldn't use it that cycle, right?

Maybe I can explain the reasoning for using a trigger shot instead of relying on an OPK, because my clinic had me use a combination of 1-2 doses of injectibles and a trigger to get my follicles to the perfect size even though I ovulate just fine.

Sometimes when you ovulate, your body can have an LH surge and ovulate when your follicle isn't at an optimal size (either too small or too large). If it's too small it may mean the egg is immature, and if it's too large it may mean it's overmature and the genetic material is past its prime. Either way, they told me it could possibly fertilize but result in a chemical or worse. Their theory was that when I had a chemical just before this pregnancy I had an immature egg fertilize. And the pregnancy before that I ovulated late and had a m/c at 7 weeks.

As for the Ovidrel/Gonal-F, they use Ovidrel with all kinds of treatments, even protocols with no other meds, just to time things perfectly!

ETA: Also I think oral and injectible meds can affect the rate at which your follicles grow so the trigger shot is like an extra safeguard that you're getting it right.

I used Clomid with Ovidrel, then Femara with Ovidrel and Femara with Gonal-F and Ovidrel (which is what I conceived on). Personally, I loved using the trigger shot. OPKs can be less than precise, and I knew from the moment I gave myself the shot, I had an approximate window of 48 hours (24-36 for O, +12 hour life of egg).

I can't answer about the progesterone since I never used it. But if you have questions about my experiences with the other drugs I'm happy to answer.

@PurplePumps: injectables contain the follicle stimulating hormone. They are MUCH more expensive than Clomid. Ovidrel is an injectable drug, too, but it's the hCG hormone.

I like having a trigger drug b/c it takes the thinking out of worrying when you're going to ovulate. For me, adding OPKs to the mix would be stressful. The shot isn't bad-I give it to myself in the stomach (I thought it would be in my thigh muscle, but I was wrong). It's a large dose, so the injection site will be sore for a few days if I touch it or bend over.

You can still ovulate before your trigger, so you should start having sex on CD 10 to be safe!

ETA: in terms of lingo, I think people usually refer to injectables as the FSH drugs, and call ovidrel/pregnyl their "trigger shot." I also take a small dose of hCG during my injectable cycles.

I've only done Ovidrel with IUI when they want to get the timing as perfect as possible. I would ovulate on my own without it, but they're trying to maximize effectiveness.

Progesterone is usually a just in case - I don't think they even have data that shows clear benefit for people other than IVF patients, but it MIGHT help so they're trying to be proactive. I'm not a big believer because I just did a Clomid/Ovidrel/Endometrin cycle and I still ended up with another chemical pregnancy (#4 for me).

Oh and injectibles is generally short for any of the FSH drugs, your ovidrel is of course injectible too, but people use it to mean the FSH they would inject earlier in the cycle (vs clomid/femara being oral).

@Happygal: Thanks! That does help. I didn't know it could be Clomid or Gonal to do the same thing. What are "injectables"? Stupid me, I thought that was the Ovidrel cause it's coming in a syringe.
@Mrs. Jump Rope: That info helps too, thanks! I've gotten a surge every month and have ovulated, confirmed a few times by blood. But if I have it.. might as well use it. At this point, whatever my body is doing on it's own isn't working so I'm willing to try any help I can get. My LP has always been 14-15 days, but I have that in my list of question to ask.

I did not use Ovidrel when I was doing unmonitored Clomid cycles, but once I switched to monitoring and IUI, they used Ovidrel to time my ovulation so it would line up with the IUI (I also had normal unmedicated cycles with ovulation). The progesterone is, like happygal said, probably just an added measure. For me, I used it during my IUI cycles because I did have a short LP, and during IVF because it was standard procedure for my clinic. If you're averse to them (or concerned about the cost), you probably don't need them.

@Happygal: @PurplePumps: I did both. And, just because you have a history of O doesn't mean it WILL happen.

I did use OPKS. It was my REs policy; it's cheaper than going in for multiple ultrasounds and blood work! They gave me a window on when to start using OPKs (it was a 4-day window). If I got a + surge I called by 7am to schedule same day ultrasound and bloodwork. They wanted to confirm In office that I'd did indeed surge. If I didn't have a + OPK by day 5, I called by 9am to schedule blood work and ultrasound the following day.

Of my three months on clomid TTC #2, I did not surge on my own once. I did get the ovidrel shot in-office on month one. It forces ovulation -- so, my follicles were really big (28/29mm) but on cd18 I still had no detected surge -- so I got thr ovidrel shot.

It needs to be ordered in advance which is likely why they're prescribing it now. It's precautionary. It sucks because it's expensive and you might not need it. I just spent $120 on novarel in November and it's still sitting unused in a box I my bedroom.

The progesterone could be due to a short LP? Id ask your RE to clarify that. I take progesterone suppositories because I've had 3 CPs.

I'm not sure about the Ovidrel, but it seems REs use progesterone as a bit of a safeguard. When I got my BFP my RE put me on progesterone supplements right away even though they said they may not be necessary... it's just an extra precaution.

Can't answer all of these for you, but here's what I do know: your Ovidrel is a trigger shot that will pinpoint your ovulation window. You don't have to take it--you could rely on OPKs (I think this is what @mrs. jump rope: did!), but using it can help you time things just right.

Progesterone supplements--I think your doctor is covering the bases. Again, if you discuss with your doctor and think your levels are fine, you don't have to take it! I have had a few LPs that have been shorter than 12 days, so I've taken it the last three cycles b/c they've involved so much work and I figured I should give it all that I can.

Gonal is a follicle stimulating drug. You're taking Clomid instead, which works to stimulate ovaries in a different way. So gonal and trigger can go together, but they don't have to.

Hope that helps!

I'm going to ask my doctor when I go in on Friday, but I'm a nut and can't stand not understanding exactly what's going on and was hoping someone here could help.

I knew my doc was going to have me try Clomid again (100mg), but I'm confused as to why I needed the Ovidrel. I asked the nurse when she told me and she said to make sure I ovulate on time, and that the egg doesn't get 'too old' in cause I ovulate late. I said ok, but I didn't really understand that. How would someone know if they even ovulated "late". I know it's late in my cycle (usually CD25-32), but last time I had the u/s, I was 14mm on Thur, LH spike on Sunday, so @2mm/day, that's 20mm. And O'd on Monday.. so wouldn't that be perfect timing to O?

Then she also gave me progesterone supplements, but I'm not sure why. Last cycle my levels at 7dpo were fine (15, unmediated), so why the progesterone prescription?

And I was reading about Ovidrel and it seem like a lot of times it goes with Gonal, which I wasn't given. When is that normally prescribed?
