@brownie: Well, you are definitely being proactive, which is wonderful. Just keep gathering information, and maybe look for some other expert opinions. I think he will get back to where he was and continue to progress, but I can only imagine right now that it is very, very hard to see him seem to lose ground.

@Mrs. Twine: it is becoming more drastic of a regression. I went to the ENT because there was some regression but today was worse. We are supposed to wait 3 months but if it's this bad I'm calling earlier. The therapist just said his intelligibility was down significantly. So said there isn't much we can do but surround him with sound.

His one preschool may be able to help because they work closely with early intervention.

@brownie: Ellie NEVER had ear infections. Never. Just some retained fluid. And the change after tubes was almost immediate. I completely understand not wanting to perform a surgery if it isn't warranted, but it seems in this case the atypical tympanogram results coupled with the regression point to it being a hearing issue, and it's likely that tubes would resolve it quickly. Not that fast is maybe the right answer here, I'm just wondering. And if he made so much progress so quickly, it almost seems to me that he's at a really receptive age/place for language right now, and so it seems so sad to have him miss out because he's unable to clearly hear the speech sounds he is exposed to. What did the therapist say, if anything? Maybe she is another person you could speak to because I imagine that she may have worked with other children with a speech delay due to hearing issues.

@Mrs. Twine: He is 3 months away from being 3 years old. He has a huge vocabulary but horrible expressive speech. He just started saying sentences longer than 2 words 2 weeks ago when he was on antibiotics for his first ear infection. Since we stopped antibiotics his sentence length and structure have remained high but his intelligibility has dropped. So now we can't understand his 4 word sentences.

I think it is the hearing but without more tests that show fluid the ENT doesn't want to do surgery. He's only had one ear infection and this one test on record (I need to find the timponography from a year ago).

Waiting and seeing and watching him regress is the hardest part. My sisters son (he's adopted so it isn't biological) actually failed hearing tests and is getting tubes for hearing for the second time now. And she is telling me she hears much of what she saw in her son in my son.

@brownie: sounds like you are really on top of things and have a good plan in place. I would definitely keep a watch on the fluid and maybe push for tubes sooner rather than later if it becomes an ongoing problem. It does sound like a fluid/inner ear issue with such drastic change though. Hopefully it'll resolve itself quickly and his speech will begin to improve again.

@brownie: Sorry for the silly age question, but I was typing before the responses stating his age were up :P

It sounds to me like the hearing might be playing a huge role.

@MommySLP: When we went it wasn't much just enough. And it didn't stop the ear drum from moving it just didn't move as high as it was supposed to. So we wait because there isn't a pattern of fluid. If he keeps getting worse I will make an appointment earlier.

From last week to this week it just got so bad and was so obvious with his therapist. Literally words he said last week that she couldn't figure out. It was so crazy and very evident.

I will do EI now because he isn't 3 and then see if he can retest at 3 for the school district.

How old is he? I bet that's scary! Ellie had a regression at about 18 months where she went from having two or three recognizable words other than mama to having none for quite some time (she did still say 'mama' but that was it). It was scary, but she had very age-typical speech by the time she was around 2.75/3-ish. And actually, by that time she had an "advanced" vocabulary.

Ellie had some fluid in her ears and an abnormal tympanogram about two years ago. She had tubes placed and her hearing levels went back to normal. Might you want to bring up tube placement with the ENT, or pursue another opinion? If you really think it is related to the fluid, it might make a big difference. Ellie had a tympanogram to follow-up not long after the surgery, and it was normal. We also noticed we didn't have to speak as loudly to get her attention and she was no longer saying, "What?" all the time.

@mrbee: The requirements are " For children aged 24-36 months, a significant delay is 50% in one area or 33% in two or more areas. " With his receptive language and ability to label almost anything it isn't a 50% delay. It is also the only delay because technically his hearing is normal. If for some reason his hearing goes down as well then we may have a case.

@brownie: I know we've already talked, but since he is almost 3, can you try reaching out to your local school district? Once he is 3, it is the school districts responsibility. I'm in CA working for a school district with little ones just turning 3 & you may be surprised with qualifications, especially with speech intelligibility (language is much more difficult to qualify for). At the very least, they will bring him in for a screaning. Other than that, if he has that much fluid in his ears I'm surprised they are wanting you to wait more than a month to be seen again. So sorry you're dealing with this!

@mrbee: Thank you. I will be calling on Tuesday for sure. I'm not really sure what they can offer though. We already have private speech therapy. And my pediatrician thinks I am crazy but today the speech therapist couldn't understand words and phrases he said just last week (like soccer ball). So it was pretty drastic this week.

@beaker: They do. I don't think he will qualify because he just isn't that bad and I know kids that were way worse (autism) that didn't qualify. It is on my to do list just in case but it seems like they only qualify those that they absolutely have to. Also the EI speech teachers (he sees them occasionally at one of his preschools) don't feel he has any issues.

So, we had another set back. This week at speech therapy, his therapist couldn't understand him. His intelligibility has dropped drastically even from just one week ago.

I took him to an ENT on Monday and there was fluid in his ears (may be left over from infection). His hearing was on the low end of normal but considered normal. They did the tympasomething test and his ear drum vibrated but not in the normal pattern. I swear this is what we had almost a year ago when we did a hearing test. So now we have to wait 3 months to see if he still (or again) has fluid in his ears).

He is still saying 4 word sentences but you can't understand the words in them. Words he said perfectly just two weeks ago. Speech therapy isn't helping much right now because if he can't hear the sounds, he can't mimic them.

I just don't know what to do right now. It is so scary to watch him regress so much. Any suggestions or ideas?
