Thanks ladies!! I bought the 21mm shields. We'll see how it goes, and I'll talk with the LC if I'm still having issues.

@jaypegs: thanks for that link!! I just bookmarked it in case.

I had problems with leaking down my chest as well, and bought these angled flanges, which I greatly prefer!

I would go with the smaller over the inserts, but I agree with pp that it's too soon to tell whether they're actually too small. Mine fits properly and aroele gets sucked into the flange a bit. With it leaking like that, it sounds like there was a break in the seal where the flange meets the breast - you might just have to press the flange on a little harder. If it feels weird when you do that, you can lube your whole breast up with lanolin or coconut oil.

I also thought the standard flanges were too big when I first pumped, so I got a set of the 21mm ones...which are too small. Sigh. I'd get the actual flanges over the inserts, though. And be prepared that once your milk comes in and nursing gets established, the size and shape of your nipples may change anyway.

I'd never heard of or seen the inserts until I Googled after reading your thread... I would be safe and just get the smaller size flanges. Inserts seem more tricky!

I used my pump last night for cervical ripening (advice from my midwife). I pumped about a teaspoon or so of colostrum (hooray), but I noticed that a lot of my areola was being sucked into the breastshield that came with the pump. After looking through the booklet, it looks like the standard size is too big. I had colostrum leaking out the sides & down my belly. ATTRACTIVE! :)

Anyone else have this issue? I saw on the Medela site that I could buy two new breastshields in their small size, but they also have 21mm (small) "inserts". The site doesn't have descriptions though, and I wasn't sure if the insert was to put inside the standard shield. It's only a $10 price difference, but I don't want to get the wrong thing.
