@IRunForFun: they don't prescribe pp1 9;( the bat because (at least as of 2 years ago) there wasn't a ton of research on it and Zantac was more tested in infants. There are also some concerns about whether pp1 have an effect on the natural flora in your stomach etc. I didn't mention here but Zantac did not work for
My first son who had silent reflux/bottle refusing. Prilosec worked like a dream. This was before some of the more recent stidies on PPI came out and with our second son the pediatric gi doc was adamant about trying Zantac x3 a day before moving on to PPI.

@IRunForFun: my pedi at the time said he wouldn't start a baby on a PPI until I think it was 3-4 months, so maybe that is why. Also, if I recall, you have to wean off the PPI, so Zantac is maybe a less complicated med to try. not sure, but those are my best guesses.

@Mrs. Blue: @gingerbebe: It's really interesting how many people haven't had Zantac work and it's still the first line of defense. I wish I'd known to ask about something else. Hopefully since DD's reflux seems comparably mild we will have success.

I have a couple of blog posts about our experience with reflux.

http://www.hellobee.com/2013/07/03/surviving-reflux-symptoms-diagnosis/

The Zantac (which you may have already discovered tastes really gross, so it's kind of hard to get them to swallow it at first) helped some, but not a lot. Eventually, they were put on Prevacid, and it was such a game changer. The first week on the Prevacid was AWFUL until it really kicked in, but then it was so great. I didn't even realize before (since they were our first), how much happier babies could be when they felt good..

Their symptoms were projectile spit up, not feeding well, fussy (but again, I didn't really realize until they were feeling better), crying when we laid them down, etc.

@azjax: It's funny because DD normally hates anything being syringed into her mouth - like gas drops - and she didn't seem to mind her first Zantac dose. I tasted it though - gross!!!!

Just gave her first dose so we will see what happens. Thanks all!!!

DS had reflux and although he wasn't a huge spitter, he was extremely fussy eater, did the burp/chew/swallow thing, didn't sleep well, and was very gassy. The ped found lesions on the back of his throat from the reflux and we started Zantac. It helped a lot but we hard a difficult time getting DS to take it because of the minty flavor, and we had to up his dose as he gained weight. We eventually switched to omperazole, which we stopped around 7 months once DS started taking more solids. The meds were a game changer and really helped.

Hi there! My DD was prescribed Zantac at our two month visit a couple weeks ago. Probably should have noticed earlier, but constant projectile vomiting, screaming at the breast, refusing to eat, general unhappiness, wouldn't lie flat, and a terrible sleeper. The Zantac has helped everything (a LOT!) except the sleep. :) Hope you see an improvement soon!

We've been dealing with reflux with my 4 month old. Her big brother had it, too. My 4 month old would pull off when breastfeeding, arch her back, and cry. She would also choke when feeding and milk would sometimes come out of her nose. We got her on some Zantac and they put her on the lowest possible dosage for her weight. It didn't do much so I went back at the follow up and asked for something else. We were given a prescription for Prevacid but it turned out that our insurance didn't cover it and it would cost $300 for a 2 week supply. We then decided to try a higher dosage of Zantac to see if that helped at all and that seems to have done the trick. We also bought a crib wedge for reflux and try to hold her upright after eating for at least 20 minutes. I understand the hesitation to medicate as Zantac can inhibit the absorption of iron and calcium. However, I know from my son that it is a relatively short term problem for most kids. Around 6 months we weaned my son off it (he had started it at 8 weeks) and he did great and we never had any issues. We plan to try to wean our daughter off it in a couple months. Once they are eating solids and sitting upright more frequently, a lot of kids feel much better.

ETA: Don't be discouraged if the Zantac takes awhile to work. With both kids, there wasn't much improvement for the first two weeks.

We did Prevacid with our first and then Prilosec with our second bc Kaiser just switched to compounding one over the other for infants. No real difference or reasoning.

My husband has horrible GERD and I'm constantly terrified he's going to develop esophageal cancer (my dad just got diagnosed and he's had GERD too), so I'm super vigilant about it with my kids. Meds aren't something to be casual about but neither is permanent damage and scarring from acid burning. My first son had so much pain as an infant he never associated milk and food with comfort. He was low on the weight scale and we worked intensely with him for the first year to get him to eat and drink enough. It wasn't until 18 months I felt like I could relax about his calories and stop putting olive oil and butter in everything. DS2 on the other hand got relief much sooner and he's been a great eater and drinker.

@IRunForFun: yeah that's why I cut it too - plus his older brother had a diary intolerance (blood in stool as infant) so I figured it couldn't hurt. I reintroduced dairy at 7 months and it wasn't an issue at all. His spit up started slowing around 8 months and we weaned him from reflux then....he stopped spitting up around 10 months.

@catlady: The pedi did say that the dosage is based on weight, so it usually needs to be upped as they grow. What does it sound like to reflux without spitting up? LO often does a gurgly burping noise and I'll be sure she's spit up but there's nothing. Is that it?

We've had a good experience with it. LO2 has silent reflux diagnosed at 2 weeks. He did not spit up much, but he would gasp and choke while nursing (it almost sounded like he couldn't breathe, which is why I brought it up to the pediatrician) and I could hear it coming up and then he'd swallow it down. He also could not sleep on his back at all. The zantac definitely seemed to help with the gasping and choking, although not necessarily the sleep at such a young age. His sleep got quite a bit better starting at 6 weeks, which was probably just developmental. At around 8 weeks or so, the prescription ran out and we let him go a couple days without it and he started choking while nursing again and he was generally fussier. So we started him back on it and at his 2 month appt, the pediatrician upped his dosage since he'd gained quite a bit of weight, and that seemed to help even more.

Right now he's almost 4 months old and I bet the dose is too small again (he started spitting up a lot more recently). But he's a happy guy, doesn't choke anymore, and can sleep on his back now, so I think it's still doing something.

DS had silent reflux and was on Zantac 3x/day for a loooooong time. He spit up constantly, was very fussy and wouldnonly sleep on an incline. He also constantly sputtered, choked and coughed while nursing. We just weaned him off at 16 months old. It was a game changer. I found the website refluxrebels.com very informative and helpful. They also have a fb group that I joined and received a lot of advice from. Also, I would get your lo checked for a lip/tongue tie, which can exacerbate reflux symtoms.

@MrsADS: It's great to see that even in very difficult cases, a combination of medicines will make the reflux improve. The pediatrician of the daughter of a friend of ours didn't think it was necessary to medicate her very bad reflux, and she suffered and didn't sleep well until she was nearly a year old.

@beehive just my opinion (and extensive experience with ALL the reflux meds for my son, ugh) there is no real difference between Prevacid and Prilosec, but Prevacid seems to be more commonly used in infants. I think maybe there is more published research/trials and it has been used longer in infants so it tends to be the first line PPI.

For us, Nexium worked better but I think it is pretty unusual to have to switch to that. We tried Prevacid first.

We also did the PPI AND Zantac, because they work differently. He was on both for a long time. We also used Gaviscon (extra strength with the red cap) for breakthrough troubles (this was after about 8 months old however and in dosages given by our GI doctor) to coat.

Oh, and as a little baby, he was also on Carafate to coat the esophagus and "soothe" the burn. He took:
- Nexium twice a day
- Zantac 3x a day
- Carafate 4x a day

If you are having continued reflux issues on meds, I cannot overemphasize the importance of seeing a pediatric GI, if it is at all possible.

Zantac didn't work when DS2 took it 2x a day. We ended up seeing a pediatric GI specialist who put him on it 3x a day (I think it was 1.2 ml x3) and after a few weeks that did the trick. DS2 spit up colossal amounts, hated being on his back and would arch and fuss and claw up my chest after feedings. With the Zantac x3, he still spit up tons, but he was a happy spitter. I also cut dairy from 6 weeks to 7 months. I don't think that really made a difference, though.

Sounds like reflux to me - agree with @gingerbebe that you've GOT to do meds, and I would be aggressive with moving quickly to a PPI (i.e. Prevacid, Nexium, omeprazole) if Zantac is not working, or both. My son was on Zantac and Nexium for over a year. You may need quite a high dose of the PPI med, as well - just keep in mind that all babies are different.

And if you are not seeing improvement, PLEASE go see a pediatric gastroenterologist (GI doc) for an expert opinion. I love our ped but to be honest, I felt like once we got past the Zantac + a little Prevacid they were sort of out of ideas. Our ped GI was the game changer.

Unfortunately I had to go on a pretty restrictive diet as well (BFing) to see some improvement.

My son was on Zantac on day 3 of life an on a PPI after 2 weeks old, failed Prevacid and omeprazole and moved to Nexium by about 2 months, and at 10 months he was on 40mg of Nexium a day (an adult dose) - that high of a dose was the only thing that finally made his reflux improve. I don't think your LO is that bad, but the meds are the difference.

I know it is scary giving your little baby meds (I hated the idea), but this is an area where it is truly, truly necessary, and I would not hesitate to give them AND push for increased dose/different meds/specialist if it is not working.

@IRunForFun: Oh yes, soy and dairy went back in before we started the zantac! (the rest are long term allergies so I'm used to them and still find plenty of delicious junk to eat!)
Also, caffeine never had any effect on LO's symptoms!

@IRunForFun: I also heard that giving up caffeine can help, but I really want to find a solution that works without making that change! I was so excited to switch back from decaf after having DD...

@Silva: Thanks! I've heard so many people talk about how it's over-prescribed and how all babies reflux, etc, but I'm kind of desperate at this point. I have heard the over-eating to soothe theory, but I don't think we have that issue. She's in the 10th percentile!

@Rocker2014: It's funny you mention the mom's group because I've gone several times to a BFing support group and noted how the other babies seemed to stay latched for SO long and be so content nursing and relaxing and then there's my DD who I had to keep latching and burping and re-latching and soothing, etc. I hope you got to add soy and dairy back in, that type of restriction sounds awful!
