My LO randomly throws up like this when she has an ear infection. I have no idea how it's related but every time she's done this we've found out she had an ear infection the next day.

@Mrs. Tiger: That's the only thing I can think of too, it just seems weird because he didn't throw up at all when he was really sick and the phlegm was out of control...but now that he's almost back to normal suddenly this. But I guess he can't really tell me if he has phlegm or not, I'm just guessing based on how his breathing sounds. I was holding off on milk when it seemed bad but have been giving it to him again, maybe I should ease up on that. Although he hasn't had any since about 10am and it's now 6pm, so maybe not. Argh.

Gross/tmi, but maybe phlegm is running down his throat, making him gag & then throw up? K has a really strong gag reflex and if he chokes or gags on something he often throws up.

I am totally baffled by this and am hoping anyone has experienced something similar or can shed some light on possible causes. The past two days in a row, L will be running around playing and acting totally fine when he suddenly stops, throws up a bit, and then runs off again like he's perfectly fine. It's the weirdest thing and I can't figure out what could be causing it or if I should be concerned. He'll be 2 in May and the only time he has ever vomited before was on Thanksgiving when he had way too much to eat, which has not been the case either of these times. He's had a bit of a cold since last weekend, but seems to be on the upswing and almost over it. Both times it happened between lunch and dinner, but his lunch was totally different both days so it doesn't seem like a particular food is the cause either.
