I am going to take a tour next week and find out who the anesthesiologist will be.  I am okay with a hospital setting, I just need her to be monitored by an actual anesthesiologist.  Ugh!  Where would your child's be done?&#60;/p&#62;

<description>&#60;p&#62;@Greentea:  so sorry to hear that :(.  Our plan was to hold off till age 3ish when he could just have mild sedation but I am definitely rethinking it. It seems like it might be more traumatizing that way because he will still have to be restrained. We have an appt coming up in spring to check on everything and try to do another X-ray. I have totally come around to anesthesia, especially as I got cavities worked on in the last year. It's just so terrible getting dental work done and I imagined him feeling how I was feeling :(

I think some families are so good about feeding their children healthy foods, but very few things have no sugar, and eating acidic foods/foods with sugar in them throughout the day can cause cavities even if it's just fruit. It's tough. Some kiddos can get away with being more lax and some can't :-(

I think we have a pediatric dentist here, but for my patients I recommend fluoride varnish at the dentist office, a low dose of fluoride tablets and a schmear of fluoride toothpaste. There is both a topical and a systemic benefit to fluoride, and I go by the ada guidelines. I find that in my area, our pediatricians have already prescribed fluoride tabs so I don't have to do it much. There are different strengths of tabs so your dentist should be prescribing the correct dose for your child's weight. If they are at a low risk for decay it's not the end of the world not to use the tabs, but if I have children, they will be taking tabs.

At our 15m pedi appt, I mentioned the markings on his teeth to our pediatrician (our dental appt was schedule for following week) and then he applied it.    Then, we had a 3m dental follow up after our initial one for another varnish application. That same week we ended up going back in to get the silver treatment, with another thick application of varnish. I see you are in Portland, a friend of mine who lives in Portland said her PMD told her to start applying a tiny amount of flouride toothpaste to her less than 1yo's gums. &#60;/p&#62;
&#60;p&#62;Oh and our insurance didn't cover the SDF treatment at all!

I am familiar with silver nitrate from my work and I thought &#34;huh? You can apply that to teeth???&#34; But then it was definitely silver diamine flouride they used. I don't go that route, because again, private practice, people are sue happy, I'll let the specialist decide if they want to treat. We don't even have nitrous in my practice though, so I only work on the most angelic children ever. It's great.&#60;/p&#62;
&#60;p&#62;Edit to add: the times that I have gone conservative with treatment it's unfortunately not worked out. There's no fluoride in the water here though and people won't use fluoride toothpaste.

We just found out that my almost 3 year old has a few very small cavities behind her lower front teeth, probably from when I used to nurse her before bed without brushing afterwards and then she'd have a pacifier all night long (bad mommy, I know).  My dad is a dentist and said there was nothing we could do at her age so we would just be very good about brushing and wait and see.  He may apply some fluoride at her next appt (6 months) and possibly may try to treat them in a year.  He also said we may get lucky and they'll just fall out before we need to do anything.  I should ask him about this silver thing.

<description>&#60;p&#62;I actually think it's probably silver diamine fluoride if it stains the decay black. (Silver nitrate is a cauterizing agent.) It actually arrests the decay over 2-3ish applications but it is not esthetic. If there is no staining then it's just regular fluoride varnish. That will not arrest decay, but help protect healthy enamel.

It's just supposed to protect the teeth, not fix it. They literally just paint it on like nail polish. I live in Sweden, so that may be the reason why it's uncommon in the States. I know I've never heard of the practice before. &#60;/p&#62;
&#60;p&#62;But I should stress that it doesn't look good. It's just supposed to buy some time for her she's mature enough to get real treatment. When my mom and relatives see DD in person (I'm from SoCal) I know they'll say something about her teeth. It looks like she hasn't gone to the dentist at all, but in fact she goes every 3 months  :bummed:

But then a different dentist in the practice called me back and told me about another option. They apply silver fluoride the then cavities and it usually halts the progression of it. It does not repair the tooth and it has a horrible side of turning the cavities black. I still decided to go for it, and now he has black spots on his front teeth :(. I just cross my fingers it will stop them from progressing or spreading! We can then repair the teeth when he's over 3 and can do conscious sedation.
