<description>&#60;p&#62;@pachamama:  All fraternal twins and 1/3 of identical twins grow separate placentas.  This is called dichorionic.  2/3 of identical twins share a placenta (monochorionic).  My understanding is that it depends on how many days past fertilization the zygote split occurs.  If it happens early enough then they can develop their own.  It is also possible for two placentas to fuse but we are pretty sure based on ultrasounds that mine are monochorionic.&#60;/p&#62;
&#60;p&#62;A shared placenta has earlier risk of degrading which translates to increased stillbirth risk.  That is the reason for the 37-week cutoff.  It also introduces risk that one twin may steal resources from the other which is a serious complication for both the donor and the recipient.  The conditions are called TTTS and TAPS.  There's about a 10-15% chance of a monochorionic pregnancy developing TTTS, so protocol is monitoring ultrasounds every two weeks.  (I think protocol for dichorionic monitoring is every four weeks, but I'm not sure since I don't have that kind of pregnancy.)&#60;/p&#62;
&#60;p&#62;With twins in general (regardless of chorionicity) you can attempt vaginal delivery if both are head down.  Some providers will attempt vaginal delivery as long as the first twin (&#34;Twin A&#34;) is head down.  It is almost impossible to find a provider in the US who will attempt vaginal delivery if Twin A is breech, though I have heard of one in LA.&#60;/p&#62;
&#60;p&#62;I looked hard to find an OB who is comfortable with the second condition and am really hoping that Baby A keeps her head down!  It is common to find one doctor in a practice who will do this, but then you are subject to the call schedule.  So unfortunately my OB search took a lot of effort this time.  I think the OB who attended my son's birth would have done it too, but she retired.&#60;/p&#62;
&#60;p&#62;About 1% of monochorionic twins also share a amniotic sac.  This is called monoamniotic (versus the more common diamniotic).  Unfortunately those babies have about a 50-60% survival rate, mom is often on extended bedrest during the pregnancy, and a csection is pretty much mandated.  I think this is primarily due to the fact that the umbilical cords can get severely knotted up since they are in the same sac.  I haven't researched it too much since thankfully my pregnancy is mono-di, not mono-mono.&#60;/p&#62;

<description>&#60;p&#62;@pachamama:  Well today I am 25w3d!  I have ultrasounds every two weeks and so far everything looks good.  I am trying to take things week by week and have a few interim NICU-related milestones in my head: 28w, 32w, 34w.  The girls have to be born no later than 37w0d due to their shared placenta.&#60;/p&#62;
&#60;p&#62;We are almost done with our construction project to add a third bedroom.  After that we can finish rearranging the furniture and getting our apartment organized.  Most of the baby clothes are sorted and ready to be washed and folded and put in drawers.  We now own a 3-row SUV.  And my work transition plan is solidly underway.  My goal is to have everything in place before my third tri begins in 2.5 weeks.  Everyone says that third tri with twins is really challenging so I don't want to plan on being productive in case I'm not.&#60;/p&#62;
