My hospital provided everything! I didn't need to bring anything except a coming home outfit, blanket for the car seat, and car seat. They provided diapers and wipes (pampers), breast feeding supplies (lanolin, nipple shield, nursing pads), everything.

They only provided a couple of diapers, no wipes, a kimono for baby, a knitted hat, receiving blankets and that's about it for baby.
Everything else I had to bring myself. Formula if I wanted it, yes I took it.

As for me, peri bottle, pp pads, mesh underwear (those were great, i LOVED those) and a nightgown for your hospital stay.

Our hospital gave us so much stuff. At one point, I had to tell my husband to leave some stuff behind bc we didn't have any room. For LO: shirts(like 10), mittens, socks, hat, pampers swaddlers diapers and wipes, extra formula, pacifier, blankets,

for me: pump parts, bottles, nipple shields, lanolin, pp pads, tissues, bottle cleaner, basin, little pads to put under diaper changes, water bottle, and even a hospital duffle bag.

I regret not bringing the extras pillows tho

Our hospital provided us a box and a half of huggies diapers and a couple containers of the pampers sensitive wipes. They also gave us a blanket because I forgot to bring one in. We also got formula, hats, etc. LO was in Special Care for a week so her basinet was stocked up and the nurse said to take it all!

Our hospital provided diapers, blankets, and t-shirts for baby. They also had pacifiers, but only if you asked for them. We ended up having to rent a pump while at the hospital since LO would not latch, and they provided all of the pump parts, including bottles. They had wipes, but they were water-activated wipes which weren't that great, so we brought our own wipes.

If you intend to formula feed, I would recommend bringing your own. We ended up having to supplement a little, and only got a few formula samples under a prescription from LO's MD. The nurses are very reluctant to give out formula.

Our hospitals in the UK don't even provide formula, unless the mother intends to EBF and, for some reason, it doesn't work. They will then provide some emergency formula until someone can bring some in for you. If you are intending to formula feed, they wont provide any! You have to take your own nappies, wipes, clothes, blankets, dummies (pacis) the works!

They provided diapers (no wipes, just paper towels and cotton balls), but we didn't use them because I'd brought my own cloth diapers.
They only gave us one receiving blanket (not to take home though), and that was pretty much it. They didn't provide any clothes, just gave her to me in a diaper and a blanket. I left her naked and just in a blanket though, because we were skin to skin almost the whole time.

I'm trying to decide what to pack in the hospital bag for LO. I'm assuming my hospital would provide diapers, wipes, blankets, etc.? I just figured I'd take a few outfits and a few blankets but not much else for LO.
