First off, sorry you're going through this. A couple of thoughts-
1. It's fairly common for doctors such as OB/GYNs and GPs to have extremely limited knowledge about fertility, so I would not trust anything they say. I would only trust what an RE says.
2. In your case, I think it's definitely time to see a fertility specialist (Reproductive Endocrinologist- RE). Don't be terrified, they're on your side and want to help!
3. Here is a quick summary of my experience- went to my OB/GYN first after a few cycles. He had zero knowledge of charting, etc (tried to say I might have O'd on CD 8 based on one increased temp, etc). Eventually got him to refer me to an RE. There I did an initial appointment, some blood work, and got an overview of the diagnostic tests and possible interventions. My RE walked me through every step and I ended up getting pregnant on my first IUI plus injections and now I have 3-year-old twins!

@Shizaz: we ended up getting referred to a specialist in December for a full work up. Because we had been getting pregnant ok, but have had multiple losses, they did tests on me first. I had to do blood tests on CD3 and 7DPO and a hysteroscopy and tube flush. My hormone levels were off so I was started on progesterone and baby aspirin for a blood clotting disorder. My DH was also signed up do a SA but we found out we were pregnant shortly before he had to do it. I'm thankful that we went and did all the testing and for us it seemed like it was a quick fix for us. I hope that your doctors can also figure out what's going on with you and that a pregnancy is in the near future!

I don't know how much help this will be but I'll just mention that even though I was getting positive OPKs my OB wanted to make sure I was getting a good follicle. So besides bloodwork I needed to come in when I got a positive OPK for an ultrasound to look at the follicle. He wanted to do this before testing DH because he said it's very common to get a positive OPK without a good follicle. But I'm no expert for sure.

@Shizaz: My doctor gave us a script for DH to go to a lab. Because he has an HMO, he looked into getting one from his own doctor so it would be covered (my insurance wouldn't cover it).

We ended up getting pregnant so we didn't go through with it.

My doctor ordered it along with a full month-long hormone panel for me, so she basically said fair is fair. I went every other day for blood draws, the least he could do was provide one sample.

@ms.line: To do this, I guess he would make an appointment with his doctor? Or should I talk to my doctor, and let her tell me what/where he should go?

Ah, I feel bad asking DH to do that, but it is much less invasive than what I will have to go through...

Getting your partner a sperm analysis is key! It's a quick, cheap, and non-invasive test that he should get ASAP, before you go down the road of multiple rounds of bloodwork and ultrasounds/HSG.

@Shizaz: HB ate my earlier reply. I did not think I was ovulating with regularity. What @marym said was true for me, my body would gear up and then not ovulate. I did temp for a few months which never confirmed ovulation.

ETA: This is what made me insist on testing by my doctors before we'd even been trying a year. I know I had basically zero shot of getting pregnant given that I didn't think I was ovulating. Also no, I had no pain from my cysts. My cycles eventually became regular at 30-32 days. And my periods were not heavy. I NEVER would've thought I had PCOS.

Based on what you said is why I might suggest charting.

You can get a positive OPK because your body is trying to ovulate, even if ovulation doesn't occur. But your temp won't shift unless your hormone levels change because it has occurred, so charting might help confirm if you are or are not ovulating.

And if you aren't, a skilled doctor will be able to use your chart to try to pinpoint problems. Your temp should be rising and falling at specific points because of your hormones changing, so if they aren't, they can better see which hormones to test first.

@MaryM: I admit I didn't give charting that big of a chance. I tried it for two cycles only. I guess I just felt like I had a handle on when I O'd based on CM, OPK's, O pain and the fact that AF would show up exactly 2 weeks from when I would guess my O date was based on all that. I will try it again though, just in case the doctor tells me to start charting - then I'll have a head start.

@mrsjd: So did you think you were ovulating before you found that out? I've wondered if it could be possible that I have cysts because sometimes I have VERY painful ovaries during ovulation. I actually went to the doctor for it one cycle because it hurt SO bad for like a week. She said it was normal and just a "big" ovulation that cycle. She didn't do any bloodwork or ultrasounds though. I get pain every month, so I can't help but wonder if there's something wrong there.

But the month I temped I did see a rise in temps around ovulation time so I thought maybe I was doing okay.

@Shizaz: You're welcome! It's so nice to have the information. I forgot to mention that I ended up getting pregnant on the second cycle of clomid, metformin, estadriol, and progesterone. I have atypical PCOS - meaning all I have are the cysts without any hormonal symptoms - never showed in any bloodwork but a doc found it in an ultrasound. And this pregnancy has been an absolute dream! I am profoundly lucky and very aware of it.

@Shizaz: The toughest thing about temping for me was training myself to sleep with my mouth closed. I have a lot of sinus problems so that was hard.

I set an early alarm when I did it, and eventually was able to go back to sleep after temping.

I've since switched to creighton charting (a very detailed method of charting mucous) and while there was a steeper learning curve, I can't say I miss waking up early!

Thank you so much for the info, this helps a lot. I want to be prepared so we can get moving on figuring out the issues. I will start charting again next cycle. I just had such a tough time waking up at the same time every day. I tend to wake up a lot in the night.

@mrsjd: Congratulations! It definitely is a tough journey, that's for sure. Such an emotional rollercoaster. I hope all goes well for you!

It could be helpful for you to start charting again so you can share charts with your doctor.

Also if they want to do bloodwork, 7 dpo draws are a lot more accurate than CD21 ones...unless you have a perfect 28 day cycle.

It may also be helpful to be able to share how long your periods are, if you spot before/after them, etc.

Your family doctor may refer you. The first thing they'll likely do is send your partner for a semen analysis. It's so easy to test whether men are having issues or not, so they always try to eliminate that first. They'll likely do cycle day 3 and cycle day 21 bloodwork to confirm ovulation. They'll also test for a few different hormones. They might also test for thyroid issues. And perhaps send you for a HSG exam. This website has a few more details about some of these common tests. http://www.rmact.com/getting-started/fertility-testing/what-is-cycle-day-3

Good luck to you! I know how tough this journey is. It took us two years to conceive my current pregnancy (37 weeks).

Hi everyone. I'm just wondering what I can expect at an appointment with my doctor about the fact I have not gotten pregnant yet. (We started trying in September of last year, so I'm nearing the one year mark). I'm 10 dpo and had a BFN this morning, so I think I'm out again.

I am sure that I am ovulating. I have been taking OPKs and I get O cramps every single cycle. I did temp one or two cycles and saw a temp increase - I hated temping though so I didn't continue it.

I don't know what the issue is. I don't know what kind of tests she will do. I go to a family doctor, so will she end up referring me to someone? Ugh, I'm actually pretty terrified.
