The blanket is usually kicked off within an hour. I figure once she can move all around and stand up in bed I'm not worried about suffocation or anything. Plus it gets really cold in her room in winter time. She's 20 months now and we haven't given her a pillow. She still tosses around so much that I don't think she'd use it. He sleeps in fleece footed sleepers though, so doesn't seem to need anything. He will be two in a month and he doesn't have a pillow yet.
