O is 6.5 months and wears a 9 or 12m Carter's, and is in 9-12 at Children's place. The footed pajamas seem to be really long but fit her around the belly. CP is pretty true to size. If your LO is in 18mo clothes right now, I would go for the 18-24 month size at CP.
