I have a cousin whose parents call him by his middle name and he was always asking them why they didn't just make that his first name if it's what they intended to call him (it was a family name thing.) I have a lot of students this year who go by their preferred middle name, but on all their official documents (ids, paperwork, class rosters, grade books) it lists their "real" name.
