Thank you ALL for your input today :) I truly felt armed with confidence going into my docotr's office today. I am so sorry that some of you still do ont have answers. It is just really saddening to not know what is wrong or what to do.

I explained everything to my docotr and she put me on Metformin (100mg). She said that should kick start O because, according to my OPKs, I am always really close, but never all the way to ovulation. She doesn't think I will need Clomid, but she said she will not hesitiate to try if I am not pregnant soon.
Also, She promised to up my Metformin dosage if I don't O this cycle, and she also said we would try Progesterone post-O to make sure I implant the egg properly into my uterine lining.
I feel good. Even if I am not pregnant this cycle, at least I know that my doctor is very proactive and is taking this as seriously as I am. I trust her...

@ladybee: ditto to the "almost" positive opks.

@pui: I suspect it is some complicated and very interesting hormonal thing. I got skinny when I was pregnant, and I put on weight breastfeeding. I felt nauseous while breastfeeding for at least six months. Tons of other hormonal stuff about me seems really weird, and I imagine that they're just a little tweaked from what is 'normal' and required for proper functioning.

Bottom line, I don't have a clue.

I stopped ovulating after a mc (got pregnant first cycle trying off bc). I had a D&C May 10, 2012 and my cycles were so inconsistent. They'd be 40 days, then 20, then 60, then 30. My doctor ran blood work and my LH/FSH ratio was off. That indicated that I have PCOS. I met with an RE who implied that I had "mild" PCOS because I was not insulin resistant and my cycles weren't 90+. 50mg of Clomid worked for me but I was bloated and had cramps. The RE switched me to Femara which I liked so much more on my 3rd cycle and thats when we got pregnant (January 2013). I wonder if it was the Femara alone that worked or the combination of Clomid and then Femara or just good luck that cycle. :)

ETA: OPKs and CBFM always gave me an almost positive but never a true positive. My RE's thought was that my body would gear up to O, try, fail, and try again.

I'm 10 months pp and, breastfeeding but I also have PCOS so either one of those things - or both - could be why for me. To ovulate and get preg with A, acupuncture and herbs were fabulous. I'm starting acupuncture again soon and hoping it will help.

@TurtleDoves: Honestly I have no idea what your cycles are doing. Mid cycle bleeding can mean progesterone issues too, but who knows.

Try not to get too worried about this... You've gotten pregnant twice, and had a healthy baby. Its highly unlikely that you'll need IVF/IUI. Its just going to take a little time to get your hormones sorted out, especially after being on BC and having a m/c. I always recommend a naturopath since I've been happy with mine (and know many others who have too). Good luck!

@TurtleDoves: Hey I am living proof that you can have ovulation problems and still conceive without IUI or IVF. All we did was timed intercourse and got PG twice. And we believe the first time didn't stick because it took so long to get a viable egg. We did Clomid, then Femara, then finally Femara + Gonal-F.

The key is not giving up so that you can find a medication that works for you, if it doesn't work right away. It was definitely grueling at the time to try a dose for days, get monitored, need another dose, more monitoring, etc until either I ovulated or the cycle just crapped out (which happened my 2nd time on Clomid. First one we had a CP.)

I didn't really want IUI or IVF either. I wasn't morally opposed to it, but DH was so grossed out by the room when he went for a sperm analysis that I didn't that *that* to be how he experienced conceiving our child. And you guys have shown that you can conceive, so I get once you get a good strong ovulation, you'll be successful :).

@ShootingStar: i'm not so much scared of the S/E. I will do anything to get pregnant. But, I am scared it won't work and then I will feel like I've exhausted my options for a "natural" pregnancy. By natural, I mean one that involves sex and ovulation, instead of IVF or IUI. Not that I am against trying those as well, I just get exhausted thinking about it. I don't want to jump the gun right now though, I am only 5 months in.
My mom and all my maternal aunts went through hell and beck to get pregnant. I was lucky with #1, but apparently not so lucky with #2.

@Mrs.Someone: if that sounds like a period to you, then would I be cd14 with abnormal bleeding or would I be CD1 with a short cycle? Just asking your opinion (and anyone else reading this).

@ShootingStar: @spaniellove: thank you ladies :) I am going to go into my appointment hoping for the best.
I do feel like I have enough history and symptoms to validate b/w and possibly a trans-vaginal U/S. I will update and what she says.
I'd be happy if Metformin was all it took. I've been on it before and I am not scared to try it. Clomid scares me in that, if it didn't work, I would feel defeated.

@TurtleDoves: EWCM generally means your estrogen is high, so having it all throughout your cycle (and during bleeding) could mean your hormones are off...? Also, if you're getting almost positive OPKs it sounds like there's something going on with your LH. It sounds like b/w and an u/s would be helpful today, but the best way to figure it out might be to monitor a cycle from day 2 or 3 as a baseline.

@TurtleDoves: I'm not sure where you are with your cycle, but you should first do CD 21 blood work to verify if you ovulated or not. Then you should do CD 3 blood work. If you're not ovulating, push for help. There's no reason to sit around trying to conceive when it's not going to happen. I will warn you though, I expected my GYN to just give me an rx for some Clomid and send me on my way and they insisted I had to see an RE for it. Which was much more involved and expensive. But effective in my case, since the monitoring showed lower doses weren't effective.

@Mrs.Someone: Yes, OPKs are never positive. Always *almost* positive. And the bleeding lasted 3-4 days last times and this time I am on day 2 already of bright red blood.
As far as O goes, even if it was inconsistent or timed badly, I think I would have caught it once by now (since end of May), we have sex almost everyday sometimes, every other day.

What should be my goal today? An u/s? bloodwork? both? I am hoping for an u/s at least, to rule out cysts or trauma to the uterus. My MC in April was at 9w along and there was a TON of bleeding. So, I am hoping there aren't any residual effects still going on.

@turtle doves, when I brought it up with my dr, she said that not ovulating is one of those things that you just don't wait to happen on its own, because it doesn't work itself out all the time. Plus, after a mc, your body can be kind of jacked up. I wasn't willing to wait a year just to see if it ironed itself out. I'd push for treatment if i were you! Mine were also in the 40 range.

Have you tried taking OPKs at all? I think annovulary cycles are usually much longer than yours, so I would guess you are O'ing, just not very consistently. Also, is how long does your mid cycle bleeding last? Definitely sounds fishy and worth talking to your doctor about...

@pui: I REALLY hope you are KU this cycle:) *fx*
My cycles vary every time. Long cycles and/or varying cycles will both indicate anovulation, so that scares me.
May 15th-June, my cycle was like 40 days long.
Beg of July-July, my cycle was 23 days long
Aug: 35 days long.
All with mid-cycle bleeding. Bleeding. not spotting....ahh
And ewcm all throughout my cycles including during period, so I am getting no indication there.

@TurtleDoves: I did use metformin and it didn't seem to do anything to help ovulation. I thought it wasn't doing anything until I had to start monitoring my blood sugar once I got pregnant. I eventually got pregnant on 7mg Femara (the highest dose) + a few doses of 75iu Gonal-F (injectable). That was the only cycle where I ovulated right away - the other ones we tried lower doses of meds and then when it didn't work upped the dose. Those cycles I was ovulating more on like CD 30+.

But again, I have *really* stubborn ovaries. Most people seem to have more success than I did with lower doses of both Clomid and Femara.

No one knows yet why I don't ovulate on my own, but if I am not pregnant this cycle (FX!!) I am getting a lot of tests done with an RE to figure it out.

So far 50mg of Clomid seems to be working for me, but I'd still like to know why I don't O.

How long are your cycles? My 140 day cycle was a big indication that I wasn't ever going to ovulate by myself. I finally O'd at the end of the cycle after taking Clomid.

@jetsa: I see :( I'm glad it worked out for you, but sorry you had to go through 18m of of trying.
I hope it is enough for her to give me an u.s and some blood work.
I think she is taking me seriously because when the nurse went to talk to her about me, the doctor came on to the phone and said "come in today", even though I was calling for an appointment to come next week. Maybe she wants to catch me before this cycle ends (I am on CD14 today).

I have PCOS and only had one normal cycle after stopping BC. I have seriously stubborn ovaries, it took several different medications to find something that worked for me.
