We brought her home in cotton pjs with a warm fleece one piece on top. And a fuzzy hat. DH was right outside the door so she wasn't outside for long. He was also 6 weeks early and under 5 lbs when we brought him home. He was in a onesie with blankets and DH warmed up the car and drove it right up to the door of the hospital so DS was only outside for less than 2 seconds. I'd say a hat, warm onesie and blankets should be enough.
