I would recommend getting it prepared by a lawyer.</p> <p>Also note that in Ontario guardianship is only temporary (90 days). The intended guardian can then bring a court application asking that he/she be appointed the permanent guardian. We got lucky and it was my moms friend so she did it as a favor. I think it was supposed to be a couple hundred though.
