I'd probably move them to your local area and use all the frozen ones first (in two transfers), only because the cost is so much less. PGS is $6000 (!!!!!) extra at my clinic, so that's over the cost of a frozen cycle, which I think are $4k each. How do the costs compare at your local clinic for frozen vs fresh, especially when factoring in the cost of the meds and any extra time off work? Does your first RE think the remaining embryos have a good chance? If you end up doing a FET, maybe read up on endometrial scratch biopsies- I've read these really help with implantation if done at the right time.

@GreenThumb: As frustrating as it is, for cost reasons, I'd use whatever frosties we had first (that way I'd not have to make the decision about what to do with 'leftovers' afterwards, too) - but it's got to be what your gut tells you. x

I have had no luck with frosties (1 missed m/c & 1 early m/c) either, and was really hoping to get a fresh transfer (my daughter was from fresh) but that didn't turn out - and I have to say, I'm TERRIFIED about using our frozen embryos. :(

@GreenThumb: ugh! Tough decision. Sorry about not having coverage when you moved to MA. :(

I know you said you prefer fresh cycles over using for frosties. Would you consider getting a second opinion in MA and seeing if there is another protocol that might work with your frosties?

i am so sorry this cycle didn't work out. big big hugs. thinking about you lots as you make a decision on how you will move forward.

at my RE's office, genetic testing is big. apparently, 90% of their patients do genetic testing and they give a 80-90% chance of success with genetic testing. i unfortunately never made it that far since they recommend having at least a couple of embryos making it to day 5 (i only made it both times to a 3 day transfer). it's definitely something to consider with your frozen embryos! it's $$, but it might be worth it to guarantee the best shot at a FET.

I am SO sorry that it didn't work this time! Honestly, if it were me, and cost was a factor due to lack of insurance coverage, I think I would have my embryos transferred to MA, and try one more FET. Given age and all that, if my next FET didn't work, I would do another fresh cycle with the new clinic. It is such a tough call. I just would hate to move to a fresh cycle costing all that money if there was a chance we didn't have to. Best of luck to you!

I'm not super knowledgable about IVF, but I'd probably be inclined to do another FET cycle and possibly genetic testing on them if it wasn't prohibitively expensive. I used IVF New England (formerly Reproductive Science Center of NE) and they were great.

Boy...that's a really hard decision. :bummed: Take this with a grain of salt, but I think, in your position, I would try to find the best RE locally in MA, and expedite having the remaining 4 frosties transferred. From what I understand, they only thaw as many as they plan to transfer at one time (since usually they're frozen in groups of 1 or 2), so I would go ahead and do one more FET with your new clinic...and if that were not successful, strongly consider doing another fresh cycle. I really want you to be successful with your frosties...but I also understand how high the stakes are. BTW-- with the new freezing technique (vitrification), I don't think there should be any "aging" of the frozen embies. Wishing you lots and lots of luck and sending good vibes! :goodluck:

@Trailmix: I know, I wish we had insurance coverage!! I will have to ask my doc about the impact of time on the frozen embies. They have been in the deep freeze since July 2012, so 2.5 years now… When we had the successful FET for LO#1, they had only been in for 3 months.

So sorry it didn't happen the second time. I'd do a fresh cycle, especially if you're in MA where IF coverage is good. I think after some time, even if the embryos look like high quality, they don't fare as well as newer ones...
ETA: Just read your responses and that stinks about your insurance! I still think I'd do a fresh cycle but it is harder...

@PurplePumps: That is interesting to know about the increase in success with screened embryos. I'm going to have to find out more about what is involved in that now that they are already frosties. PGD wasn't something we even had to think about up until now, so I know almost nothing about it.

@Shutterbug: I did end up doing my u/s monitoring here locally and really liked the doctor, so I definitely think we will get a second opinion. Doing anything locally would be a lot easier, although my WI doc did not recommend having the 4 remaining moved saying that their lab know the embies best (but in the back of my mind I wonder if it is just because they would lose our business).

@bhbee: Thanks! I think we will definitely get a second opinion locally. That is one huge positive (among many) of moving to the Boston area - great doctors and medical facilities!

@brownie: That is good to know that you were more successful with a fresh cycle than frozen (congrats BTW!). I do agree about them being young embryos being a good thing… they are 33-year-old rather than almost 37-year old. I did ask about chromosomal testing after the 1st CP, but my doc said that nothing indicted it, that CPs just happens sometimes. We'll see what he says this time. There was 0 HCG in my system, so I don't even think we even had any implantation.

A FET cycle would be a lot cheaper than fresh, so I would be inclined to use the frosties that exist already and find out more details on cost genetic screening and what exactly the embryologist thinks of the quality of the remaining ones. Even perfect looking embies look cell wise, can still be genetically domed for failure. My RE was tossing out figures of 50% of embryos that are genetically bad I think. I was reading an article (wish I could find it now) about genetic screening and with screening and known genetically good embies, success rates of IVF went from 50% to upwards of 70-something%.

We had a successful IVF followed by two unsuccessful (chemical pregnancy) FETs. I know people say that FETs are just as successful and possibly even more successful but that wasn't my case. We went down a completely different path that we haven't discussed online. I am currently pregnant with our second baby.

With 4 embryos left, can you do chromosomal analysis on them? Since that is what is supposed to be the reason most of these fail. Technically, these are young embryos (from when you are younger) so they should be good embryos.

I'm sorry your FET didn't work this time :( I think if it were me, I'd have the remaining 4 embryos moved to a MA clinic (do you have a clinic yet?) and do FETs with the remaining ones locally. Maybe not having to travel will help you to stay calm/relaxed (I SWEAR I'm not telling you to "just relax"! :silly: ) A new clinic might also have a different way of prepping you for your FET - are you doing natural or medicated?

Wish we had coverage - definitely would make the choice easier. We don't even know if we should try FET again. Feels like we have paid so much over the past three years… when do we call it quits?!

do you have ins coverage under the MA mandate for IVF? that might be a factor in how to approach.
I never got to the IVF stage so I'm not the best to give advice but it seems like embryo quality is not always related to success rates so might be worth trying the rest of your embryos if a FET is much easier on you than fresh?
I do know at my clinic in MA they pushed for single embryo transfer since you're not paying as much per cycle given good insurance coverage (usually).

We've just experienced a second FET failure in a row and are wondering what to do next. What would you do? Any suggestions?

Backstory: I have a blocked tube and DH has MFI, specifically with morphology. Because of our ages at the time, we decided to quickly move to IVF in order to maximize our chances of a BFP. Our fresh cycle was a bust (1 embryo transferred, mild/moderate OHSS for me), but we did end up with 10 frosties. Waited a few cycles and then had a successful FET #1 (2 embryos transferred, 1 wonderful baby girl  ). Fast forward to trying for LO #2, we have 8 frosties to work with. Go through the FET #2 (2 embryos transferred - 1 great one, one so-so at the advice of my doc who was confident that we would have twins if we transferred the 2 best) end up with a chemical pregnancy. Then we move from Wisconsin to Massachusetts. Our remaining 6 embryos are still in WI, so we decide to try again at our original clinic. Again had what looked like a perfect FET #3 cycle and transferred the best 2 embryos this time. This resulted in a BFN.

So what do we do now? We have 4 remaining embryos in WI. Should we go back one more time? Have the embryos moved to MA? Genetic screening on the remaining 4 (this was not indicated by the doc before this last transfer - I asked)? The 4 remaining are still "good" embryos from what I understand, but we've already had failure with the "best" ones.

Or do we try a whole new fresh cycle in MA? I am 36 and DH is 39, but are both soon to have birthdays (ouch). Any additional diagnostic testing?

Any thoughts? Thanks!

ETA: Even though we are in MA, we do not have insurance coverage because DH's company is headquartered out of state and opted out.
