My mom eventually asked us if we would prefer she buy gifts that same way as always (a bunch of stuff, mostly cheap) or buy one or two nicer things. My brother especially latched onto the idea of one or two bigger things quickly, he much prefers getting one thing he really wants.

What if you make a game of pretending you have $200 how would you spend it while looking through a toy catalog or website so he can see the prices? If that game goes well you could spring board into a birthday conversation and if not you can just handle it as you see fit this year.

I love talking my nanny kids shopping for birthday gifts for their siblings! We usually stick to a pretty low budget but they typically find something I would never ever buy but turns out to be much loved by their sibling.

As DS gets older we tend to buy one "big" gift and a few smaller ones. We don't have a set amount, but we definitely don't go crazy either. I also like the idea of "want, need, wear, read" which is what we did for his birthday last year.
