Love breastfeeding and have had no problems.
Now my left breast has stabbing shooting pains that radiate down my arm.
It's Friday night here and so won't be able to see a doctor easily. I do not want a massive problem to take hold..
What should I do?! What should I be looking for?
Please help I'm petrified!

I agree I would look for signs of thrush. This was my only symptom.
You can start by washing everything in hot water with vinegar.
You can even swab off with some vinegar on your breasts.

How would I know if its trush, mastitis or a blocked duct..? How would a doctor know?

If you don't have a fever or pain I would think its not mastitis but I never had that.
Shooting pains like shards of glass particularly after nursing was my sign of thrush. That and some itchiness.
If your baby has thrush, the inside cheeks will be coated in white (more then the normal milk white).

You can check for a clogged duct by making sure you don't have a white spot on your nipple. You can try to pop that or open it up if needed. You can also massage with warm compressess to work it out.
