Hi! My DS has been through four milk challenges so far: powdered milk in baked goods, regular milk in baked goods, baked cheese (lasasgna, pizza, etc.), and finally stovetop mac n cheese. That was just last week. The way they do it at our allergist's is he got about a tablespoon (or large bite) then waited 30 minutes; slightly larger portion then 20 minutes; another portion then 20 more minutes. Every time the timer went off, the doctor came in and listened to his lungs, checked his skin, etc. We've gone through the same process with baked eggs. He's passed every challenge. On the day he passed the challenge he was restricted from the food for the rest of the day, then we had to ensure he had a portion of it every day going forward.
Anyway, you can barely consider DS to be milk allergic now and he had an anaphylactic reaction as an infant. He's still too reactive to eggs on his skin test to do stove top eggs but he can eat them in baked goods. Go to the doc's armed with entertainment! Those long appointments are killer, not to mention it's hard to explain to a toddler why he can't have another bite just yet. Good luck to your DS!

Sounds like, regardless, it will probably be a long day, so thanks for the heads-up! So glad to hear your son has done really well with all his challenges! I hope mine does well too.

We didn't do a baked milk challenge, but we did do an oral challenge for avocado at our allergist's office. Our daughter had a positive skin prick for avocado after quickly vomiting a couple times after eating a ton of it, even though she'd eaten avocado tons of times in the past. They did the skin prick to be safe but allowed us to do an oral challenge even though it was positive since she had eaten it so many times previously without issue. On the day of our oral challenge, they did another skin prick to make sure the result wasn't more significant than the prior skin prick. The skin prick on the day-of was actually no longer positive! It was "unequivocal", but they still wanted her to actually eat the food. The first thing they did was swab a tiny bit inside her cheek to see if she started itching. We waited 15 minutes. Then, they gave her a tiny, tiny bite. 15 more minutes. Then they gave her 1/2 a teaspoon. 15 more minutes. Finally, they gave her a full "serving size" of 2 tablespoons. At that point, they checked her every 15 minutes until 2 hours had passed without any kind of reaction. At every one of the checks, they looked her over for rash, runny nose, increase/decrease in temperature, and blood pressure. They also listened to her heart. And of course they made sure she didn't vomit.
It was a really, really long day. We were there over 4 hours. But, I'm so glad we did it and that we know she is not allergic to avocado. The reason we saw the allergist in the first place was due to possible FPIES. We actually have an appointment with our allergist next week to discuss possibly doing a baked milk trial. I'll update if he gives us anymore details about what it entails.

My coworkers son did one and it started with powdered milk. I don't know all the details other than it was a looooong process but it was very successful. He's severely allergic to basically everything. Poor kid!

@catomd00: Thank you! Sounds like it's common to start with powdered milk first. I hadn't even considered that, so I'm glad to hear what others have done.

Just wanted to update this in case anyone else is searching for information on the Baked Milk Challenge.
Xander has his today. I had to bake him a cupcake using milk and bring it in. The nurse then cut it into sixths and he would eat a piece, wait 20 minutes, and then she would check his belly, back, and breathing, and then he would eat another piece. After finishing the last piece, she waited 30 minutes and then checked him again. He passed!
Unfortunately, we still have to be really conscious of milk in items. He's now allowed eat store-bought baked products as long as milk isn't one of the first two ingredients. He can also have home-baked products as long as they are thoroughly baked and a serving has no more than one-sixth cup of milk. He still can't have things like frosting, cheese flavoring on crackers, milk chocolate chips, pancakes, pudding, etc.
