We bought one that doubled as a mattress protector but it didn't fit on our mattress for some reason, so we returned it and have one of those mats that lay under the crib sheet. She seems fine with just that

They are safe but as to your needs for one, that likely depends on your type of mattress. We have a Moonlight Slumber that doesn't need one since the seams are sealed so there is no chance of anything getting to the mattress itself.

same as @pelikila -- the moonlight slumber ones are totally waterproof, so I didn't bother with anything under the normal sheet.

I got one out of habit (we have them on our beds too) to protect the mattress, but we totally didn't need it because the mattress itself is waterproof. But it's definitely safe.

We have a waterproof cover but there isn't much padding to it.

We have a thin, organic, waterproof pad. I wouldn't be comfortable if it didn't fit tightly or if it were too thick or soft.

We have one. It's not very thick or soft, but I think it makes her a little more comfortable. We have one for the p'n'p when we travel too. I think that one probably makes more of a difference on the thin mattress.

We have a thin waterproof pad we keep under the sheet (DS has never slept in his crib though). We did the same for the PNP.
We did purchase a thicker organic mattress pad, but once it was on the bed the mattress no longer fit the crib! We washed it before testing it out so couldn't return it.

We bought one, I washed it last night and now I'm crossing my fingers that it fits when I put it on the mattress tonight.

We have one! The pad doesn't have much padding to it and it keeps the mattress waterproof for our boy who used to pee through everything!

We have one, I think it makes it a bit more comfy but it's not tooo cushy where it would be unsafe.
