The nurse that was assigned to me asked me what I had envisioned for myself and I just told her. But my "birth plan" was very generic and straight-forward.

I had one, but when DH pulled it out when they were having me sign consent forms for the epidural I needed like I needed my next breath, I waved it away. At that point there was already no way my labor and delivery was going to be anything close to my ideal, and I decided just to go with it and not worry about a plan.

i did. My midwife requested me to have one. And then I ended up being transferred to the hospital so I'm glad I had it on me.

They asked me for one and I didn't have one to give them. At our hospital class, they gave us one to fill out. I told the nurse, "I don't care as long as I can get an epidural, do delayed cord cutting and you let me hold my baby right after she's born."

I printed one out, but never gave it to the nurses. I had an awesome nurse who just asked me if I had any special requests. I told her, and she very graciously followed everything I mentioned to a T.
She even stopped my OB (scalpel in hand) from giving me an episiotomy, because she had remembered I told her I didn't want one. In hindsight, I really regret that because I ended up getting 30+ stitches from tearing so badly.

I wrote one out that DH and I discussed so that we were on the same page when confronted with decisions, but I didn't give it to anyone. My OB and I discussed my preferences, as well.

My OB requires that if we have one, to fill it out and submit it when we register for the hospital so they already have it in my file. I believe preregistration is at my 36w appointment. I don't know how much it'll get used...it's more like a reference.
Mine is pretty simple: epidural ...Give me baby ASAP. Thanks!

I wrote things down to tell my midwife, she put them in her computer for delivery day.

I gave mine to my midwife at one of my appointments. She put it in my file, but I brought copies to the hospital anyways. My labor went so fast though that I never even pulled them out.

I am so mad...I forgot to ask on my hospital tour if people really do birth plans here. I think there is a lot of informality and nothing goes down on paper. I am the type to forget something though and will probably put it on a note in my phone!

Not with either one of my DD's, and I won't this time either. My MD is the one who delivers and he knows us really well. We have talked about it, and I feel like I am definately able to tell the nurses my preferences. So I have never felt it was necesary to have one, Maybe if my doctor's practice worked differently I would have a different opinion.

I printed mine but because I had an "emergency" induction and then c-section I never got to use it.
