I would definitely get a new mattress. Unless it has a plastic cover over it!

Hmmm...I probably would get a new mattress. I don't think crib mattresses are made to last that long. Even adult mattresses have what, a 10 year life span? That being said, it may be fine and you'll be putting a cover/pad on it anyway.

I'd get a new one, only because kids tend to stand up in them a lot and I could see the mattress getting worn down.

I would get a new mattress too. I found this on the Livestrong website:
"Baby mattresses are also subject to recalls and have been improved to better fit in cribs that reflect newer safety standards. As with adult mattresses, baby mattresses can harbor bacteria and bodily fluids if there is a puncture or tear in the waterproof plastic that houses a baby mattress."

Our mattress is actually one given to us by my SIL's sister. I imagine it's got to be at least 5 years old since their youngest is about that age. But it's got a waterproof lining and it's still very firm. And I also bought a waterproof mattress cover as well.
I know they're very clean people so I'm not bothered by it. So I think it depends on the actual mattress.

I would think about getting a new one just because mattresses can carry a lot of dust and dirt over time. A little baby could be very sensitive to that.
