How long did this person work for you, what was their reason for leaving, would you hire them again, were you happy with their performance, was there ever any issues of concern, did your children ever have any complaints about them, would you recommend this person, were they reliable...

I like any questions that you can't really give a vague answer for-- what is one thing you wished she did differently?
How often was she late or did she cancel? (this one is good because someone might say "very rarely, only once a month or so" and in your opinion that might be way too much)
What types of activities did she do with the kids?
What were her job responsibilities? (I like this because once a nanny I interviewed had majorly hyped up her previous job and it turned out it was a very simple job, plus you can get an idea of what types of things she might be open to doing for your family.)
