Its been kind of nice here in NYC and my daycare has been letting the kids out in their play area. I keep forgetting to ask if they are allowed to put sunscreen on DD.

They reapply in the afternoon. We put it on in the morning.

Yup. They ask that we do the first application of the day, and they reapply at least once, or more if they have water play during the day.
ETA: Not sure if it matter, but we area asked to supply the sunscreen!

yes - they sent home an authorization form about 2 weeks ago - I returned it signed with a bottle of sunscreen
I am expected to apply it in the morning and they do it in the afternoon

Yep. We had to sign a waiver and send in a bottle, and we're asked to put it on in the morning and they reapply in the afternoon.

We've signed a waiver and they apply every time they go out. We don't apply in the morning.

We have to sign an authorization form and can choose to have them use the classroom sunscreen or to supply our own.

I'm in NY as well and yes they do. I believe I signed a release form in the beginning. They put it on whenever they play outside.
We bring our own which I prefer!

Ours requires a signed parental waiver and they apply all sunscreen (i.e., I’m not expected to apply it to LO in the morning or anything). In the infant and toddler rooms, we had to supply our room sunscreen. Now that LO is in preschool, the school has sunscreen which they will apply to LO, or we can bring our own. (I’m not sure if this change happened to take place for the entire school when we entered preschool, or if it is only for preschool+).
They do restrict which sunscreens you can bring in because some of the ingredients like sunflower oils found in many of the most popular natural sunscreens have caused both a student and a teacher to have serious allergic reactions requiring ER visits.

our preschool (2 years old) does not. it's one of the things that i really don't like about it and i plan to ask about again next month when they change classes.

Yes. We have to put it on in the morning and we signed a form that allows them to put it on in the afternoon. Our daughter is SUPER fair so she needs it!

We do mornings, they reapply in the afternoons when they go out

Ours applies it in the afternoons only. It has to be brought in by parents (no school-supplied sunscreen), can't be used after the expiration date (which I didn't know was even a thing), and they only do lotions - no sprays.

Ours asks us to apply in the morning but they also apply it in the morning and afternoon. We had to sign a waiver and the sunscreen, plus they checked to make sure it wasn't expired.

At our daycare you have three options - daycare sunscreen, your own sunscreen or no sunscreen. Any sunscreen has to be the spray type, which I'm not a big believer in. So we opt out. The outdoor play spaces at our daycare are quite shaded and they're not outside for more than an hour. Also, the kids aren't supposed to be sleeveless and they always wear hats so I'm pretty comfortable with my decision.
Sometimes I'll put a mineral based sunscreen on her in the morning if it's going to be super hot.

@JennyD: honest co has a mineral spray! it's super white, but i would rather have her protected!

@JennyD: so weird, both of the daycares my kids have been to ban spray sunscreen.
We apply in the AM, and they apply in the afternoon. We had to supply the sunscreen and sign a note giving them permission. They apply it while wearing rubber gloves (I asked b/c L has some poison ivy and I didn't want to encourage the spread...)

We filled out a release allowing them to do it, sent in the brand we preferred, and they apply it before the kids go out in the morning and afternoon. I've watched them apply it... the do it with rubber gloves on and do a pretty good job.
