I was waiting to update this thread until I was feeling more confident but then there was another twist today.
I got another bfp about a month ago. Today I went in for a 7w6d scan... and we saw two heartbeats.
My husband and I are now trying to figure out how we are going to rearrange our lives

@travellingbee: It was our fifth round of clomid and our third IUI. We had actually switched insurance effective 1/1 because my husband's plan covers one round of IVF so that was going to be our last attempt before throwing in the towel.

@ElbieKay: wow that’s awesome. We also got pregnant the cycle right before starting IVF. We were waiting for CD1 to start our protocol and lo and behold it didn’t come! It was a clomid/trigger cycle without iui.

@agold: Yes, I went for a followup high res scan and they confirmed that they are mono di and identical.
@travellingbee: It's ironic bc my insurance has much better maternity coverage but doesn't cover IVF. So now our medical costs for this pregnancy will be much higher, but still lower than IVF would have cost. And assuming this pregnancy is sticky, this means I won't have to go through with IVF.
@PeaceLily: Thank you! It's still early and now it's a high risk pregnancy, but hopefully this will finally be a success story

@FoolsintheRain: Well, I am 15w3d today. So far everything looks good, and we learned about a month ago that they are girls. (I would have been happy either way but was a little excited for girls simply because my older child is a boy.)
I am almost done with a month-long break from dr appts. This coming week the every-other-week monitoring will begin.
We also put a deposit down with a contractor to convert our L-shaped living room to a third bedroom.
So now we are just hoping for a smooth pregnancy and uneventful delivery.

@pachamama: Well today I am 25w3d! I have ultrasounds every two weeks and so far everything looks good. I am trying to take things week by week and have a few interim NICU-related milestones in my head: 28w, 32w, 34w. The girls have to be born no later than 37w0d due to their shared placenta.
We are almost done with our construction project to add a third bedroom. After that we can finish rearranging the furniture and getting our apartment organized. Most of the baby clothes are sorted and ready to be washed and folded and put in drawers. We now own a 3-row SUV. And my work transition plan is solidly underway. My goal is to have everything in place before my third tri begins in 2.5 weeks. Everyone says that third tri with twins is really challenging so I don't want to plan on being productive in case I'm not.

@pachamama: All fraternal twins and 1/3 of identical twins grow separate placentas. This is called dichorionic. 2/3 of identical twins share a placenta (monochorionic). My understanding is that it depends on how many days past fertilization the zygote split occurs. If it happens early enough then they can develop their own. It is also possible for two placentas to fuse but we are pretty sure based on ultrasounds that mine are monochorionic.
A shared placenta has earlier risk of degrading which translates to increased stillbirth risk. That is the reason for the 37-week cutoff. It also introduces risk that one twin may steal resources from the other which is a serious complication for both the donor and the recipient. The conditions are called TTTS and TAPS. There's about a 10-15% chance of a monochorionic pregnancy developing TTTS, so protocol is monitoring ultrasounds every two weeks. (I think protocol for dichorionic monitoring is every four weeks, but I'm not sure since I don't have that kind of pregnancy.)
With twins in general (regardless of chorionicity) you can attempt vaginal delivery if both are head down. Some providers will attempt vaginal delivery as long as the first twin ("Twin A") is head down. It is almost impossible to find a provider in the US who will attempt vaginal delivery if Twin A is breech, though I have heard of one in LA.
I looked hard to find an OB who is comfortable with the second condition and am really hoping that Baby A keeps her head down! It is common to find one doctor in a practice who will do this, but then you are subject to the call schedule. So unfortunately my OB search took a lot of effort this time. I think the OB who attended my son's birth would have done it too, but she retired.
About 1% of monochorionic twins also share a amniotic sac. This is called monoamniotic (versus the more common diamniotic). Unfortunately those babies have about a 50-60% survival rate, mom is often on extended bedrest during the pregnancy, and a csection is pretty much mandated. I think this is primarily due to the fact that the umbilical cords can get severely knotted up since they are in the same sac. I haven't researched it too much since thankfully my pregnancy is mono-di, not mono-mono.

Well after everything I shared on this thread, I am thrilled to share that Juliet and Amelia were born on Saturday. We had a smooth delivery and are expecting to be discharged today. I am thrilled and relieved that we have had a c-section- and NICU-free delivery!
