Any idea about this!? Something sbout being grandfathered in an didn't have to abide by the ACA!

That is correct. If it's a grandfathered plan they are not required to abide by the rules of ACA. Her choices are to change plans or to pay for the breast pump. In my experience, it usually makes more financial sense to just pay for the breast pump OOP.

I don't have that exact plan, but do have United Healthcare (choice, not choice plus) and know that we do have breast pumps covered.
Does she have access to her plan info online? After logging in, I found information listed by clicking "Look up my benefits" and then "special services"\
I do know that it's still possible to not fully meet ACA if an employer picked a plan at the very last minute before the mandate passed (my employer did this, and BCP are still not covered and it's legal). So there is a possibility. But it seems like an odd thing to not cover...

United healthcare can have very different plans based on the company so it's really employer specific. I had UHC-company X plan when I had E and they covered a Freestyle.

I bought mine out of pocket two years ago, but I was able to use HSA funds. We also had flex spending account funds that would have covered it as well. If she has either of those options that should at least make her purchase tax free.

My BCBS plan was grandfathered in too - no pump coverage (even though everything else is literally covered at 100% with no deductible). So I clearly didn't change insurance over this.
On a different note, I am an onc NP and United is my least favorite insurance company. They always try to deny scans/chemo regimens etc. They just suck.
