We use a center daycare and the sick policy is pretty strict. I am wondering if it is the same for child care in a home, like with only a few other children?

As a nanny I watch sick kids, make calls to the dr/bring them in as needed. The only time I don't is for 1st appts with a new dr because parents need to sign permission to treat & give me permission to have the kids treated. But that's only come up once.
Eta: just read your post again & I think I missed the nanny share part the first time I read it! Sorry! When I did a share it fine as long as they weren't vomiting or a bad fever, most of the time they were around each other right before symptoms showed up & for a lot of things that's when you're most contagious so at that point if they were going to get sick there wasn't much we could do about it.
