If you had PPD with #1, how did you do with #2? Did you do anything differently to try to avoid developing it again?

I had horrible PPD with my son and am due with #2 in about 4 weeks. I've been feeling great all pregnancy but am starting to get very nervous about developing PPD again. I see a therapist regularly and have stayed on a low dose of Zoloft through the pregnancy, with plans to automatically increase it after she's born. But I'm looking for any other tips that helped avoid (or at least lessen) PPD the second time around. I'm hoping just knowing that it really does get better will help a lot, but maybe that's wishful thinking
