If your daycare participates in a food program, did you have to fill out a form that asked you these types of questions? I'm uncomfortable giving this information to my daycare center as I feel it is none of their business. We also pay full price to send her there, we don't get subsidies.

is there a requirement that you make a certain amount more than daycare tuition? Like, for renting we have to make 2.5x our rent to qualify for housing. Does yours require that?

@Mrs. Pen: no one ever mentioned anything like that to us. We changed daycare centers recently and this is just another in a string of forms to fill out.

@ladybee: @winniebee: exactly my thought. I'm paying full price for you to care for and feed my child. You don't need to know what I make, no matter what you feed my child or who pays for it.

@mediagirl: at our previous daycare we had a form like this. They were part of a government food program (where the daycare received subsidies) and we could refuse to provide the information. The daycare also had many children at the facility that had government funding for their daycare expenses.

Did they specifically tell you that section was required? I assume that's for families receiving government assistance and because you are not that you wouldn't need to fill that section out.

Yeah that would really bother me. It should NOT be a state requirement if you aren't receiving any assistance. That's wrong.

Pretty sure it's a state requirement. The parents have to provide the info for the program that I teach, but it's public and state-funded.
ETA: I think they have parents fill out the info because if at least 25% of the children are below a certain income level, the center can be reimbursed for some of their food costs.

Yep, we had a form like this. Same as what @MamaG: and @lilteacherbee: mentioned. It was a state funded program, so they needed to provide this kind of information. But we could opt out I think.

Does your daycare charge you on a sliding scale?
I'd just skip it personally and turn it in like that. If they brought it up, I'd just tell them I'm not filling it out.

We had to fill it out when we were in daycare and paid full price in a high cost area. We went to a large center and they provided breakfast, snacks and milk so it had to do with that. I just rounded down and didn't think much about it.

Yes, it had to do with them qualifying for subsidies from the government for "healthier" food choices. But you have a choice to fill it out or not. The income bracket of the parents makes a difference for their eligibility.
We did not fill it out.

We didn't fill one out, but that looks like the Free and Reduced lunch form we send home in public school. It usually helps with getting certain types of funding. I'd feel inclined to fill it out, but would probably ask why they need that information.

We have to do it. Same reasons others stated, the subsidies. Fair amount of lower income in our hood.

I think we had to fill one out so the daycare can get food subsidies. I'd rather they get the subsidies to try to offset the cost than to be concerned that they know our income.

We had a form to fill out, not as detailed as that one. However, if you made more than the cutoff level, you could just write something to the effect of "above cutoff" and didn't have to provide actual amounts. Like others have said, subsidies are based on the percentage of kids under the cutoff, so the center needs to know whether each family is in "column A" or "column B" but doesn't need any additional info if a family is above.

@mediagirl: We were supposed to do something similar for the daycare we almost sent lo to. But when we told them we didn't meet the actual income qualifications they said not to bother. The daycare would have gotten it for her anyway and whoever does income qualify submits the paperwork. I have absolutely no idea how it really worked..we didn't end up sending her.
