DH Urologist Referral

Hi all. DH and I are new to this whole infertility process. We've been trying for 1.5 years and are now seeking help. DH recently had three SAs. When the doctor called him back they wouldn't tell him the results, just that they were referring him to a urologist. Long story short, I went in to see my OBGYN specialist (at the same hospital) and she was able to track down his results. My OBGYN said his morphology was abnormal.

My question is, what should we expect at his first appointment with the urologist? Based on the results my OBGYN was able to obtain from the SA she recommended IUI for us. Do you think the urologist will be on board with this or want to try something else with DH?

@winter_wonder: What was his morphology percentage? A lot of girls have talked about morphology on here. I'm totally not an expert so I'm not even comfortable opining on this subject. But why don't you look into Fertility Blend for Men? Its supposed to help with morphology amongst other things.

Check to see if the fertility blend has selenium and N-acetyl-cysteine in it. There was an article in the Journal of Urology which said it helped morphology.

My DH had to see a urologist (an urologist just looks strange?). The urologist talked to him, did a physical exam and an ultrasound of the area. My DH had low morphology, low motility and low...something else due to a varicocele. We tried IUI twice (in one cycle) but ultimately IVF with ICSI worked for us. Good luck!

@mrsmcd: My OBGYN wouldn't give me exact numbers (or let me have the print out) I think this was probably due to the fact that she had to do a bit of detective work when locating the results of his SA (DH hadn't even heard the results yet). We'll find out exact info about percentages with the urologist on the 11th.

@Espion: Thanks for the article, it was an informative read. I also ordered some Fertility blend for men fingers crossed that helps!

@Swedishfish: I appreciate your info on what to possibly expect at his appointment. I'm glad to hear that IVF with ICSI worked for you guys

@winter_wonder: Our family doctor who ordered DH's SA suggested that we consider seeing a urologist for 8% morphology if my obgyn agreed with that recommendation. My obgyn said no need as she wasn't concerned about 8%. Please keep us updated on what the urologist says. I'm still contemplating whether my DH should go.

My DH was referred to a urologist since he had no sperm in his SA. The urologist did a physical exam and eventually a testicular sperm extraction. I know that's not at all your situation, but IVF and ICSI worked for us on the first try, too. Hopefully an IUI or the fertility supplements will do the trick for you guys!

Thought I would provide an update based on DH's urology appointment today. Again, I feel like I'm left with more questions than answers.

So...the urologist thinks that overall DH's 3 SAs are fine. This is contradictory to what my OBGYN thought (she was concerned about morphology).

I was finally able to get my hands on the actual report. Here are his numbers:

SA1...SA2...SA3
count: 40.3; 151.4; 155.2
motility: 70; 50; 65
morphology: 11; 38; 28

The urologist is more concerned about getting testing done on me. At this point he isn't recommending any more appointments with DH but he did have the following advice for him:

1. continue taking a multivitamin
2. begin moderate exercise (i.e., walking). Interestingly, he said that starting an intense exercise program would be too much stress for the body right now (since DH hasn't been regularly working out) and could be more harmful than positive to his sperm.
3. eat a healthier diet- full of antioxidants.
4. he also said ProXeed (male fertility supplement) has been helpful to others in the past. I just Googled it and it looks like it's about $130 for a two month supply.

So...at this point I guess I'll wait for additional testing on my end. I have an appointment next week to check to make sure I'm ovulating, thyroid, etc. Based on temping and previous use of OPKs it looks like I'm ovulating...fingers crossed.

@winter_wonder: I am not surprised that the urologist thought the SA's were ok. The first sample was a little low across the board but the other two look good. It won't hurt to make the changes they are suggesting for your DH but I agree that the priority should be for you to have a full work up done.

@winter_wonder: I think motility and concentration are the two most important factors. The new WHO guidelines for semen analysis greatly downgraded the importance of morphology in attaining pregnancy. But non-motile sperm or not enough sperm definitely preclude pregnancy (and lead to a quicker recommendation for IVF).

By current WHO guidelines, even your DH's first SA looks great.

ETA: So your OB needs to look at the new WHO data to get current.

Yeah, that's very strange that your ob was concerned! Those numbers look.great to me! The count was low the first time, but looks great the second and third samples. Good luck as you continue!

@mrswin: I feel like my OB totally freaked me out! I'm getting a bit more nervous for my appointment next week, but I keep telling myself that with each step we take it's at least more information.

@Espion: She'll receive the full report from the urologist and I'm also going to ask her what criteria she based it on.

@winter_wonder: No offence but you may be better off going to see an RE at this point especially since most RE's will want to re-do anything an OBGYN has done (at least in my experience)... plus they are experts at getting you pregnant!

@winter_wonder: Hi there, I also agree that you should see an RE. Over 1 year the recommendation is to see an RE. As stated above, they are the experts in getting you pregnant. Do you have coverage and one in your area? I think you'd be best going there and having the appropriate testing done. Blood work, checking to be sure tubes are clear, etc. Good luck!!

@jenn23: I need to look into it a bit more to see if my insurance will cover it. But, the closest RE is 1.5 hours from my house Shoot!
