This is how it's laid out here:
Elementary: K-4
Intermediate: 5-6
Junior High: 7-9
High: 10-12
Used to be 5 was elementary and 9th grade had it's own campus, but they changed it all around.

Most schools here followed the typical Canadian P-6 (I'm Nova Scotia, so it's Primary not Kindergarten) until the last ten years or so.
With aging populations in some districts, and huge growth in others, the school boards have had to adjust their popuations to fill some buildings and relieve the stress in others. So now, we see SO many different combinations, althoough we typically still call it Junior Might regardless of it's makeup, although there are exceptions.
Elementary: p-6 p-4 and p-5 can all be seen!
Junior high: 7-9 7-8 6-9 6-8
High:10-12 9-12
Several places have Elementarys that go to 8 or 9 and even 12!
Some High schools start at 7 or 8
It's all about the demographics.

We only have middle schools here. We have two campuses that are just 6th grade this year and I think it is fabulous. I hate the 6th grade transition to middle school because they get exposed to too many things that they shouldn't from the older kids.

We only have middle schools here, but the high schools just started separating out the 9th graders entering high school into a separate building. I'm guessing it's to ease the transition process.

In NY there are mostly middle schools and I'm actually a teacher in one too.
For me, middle school isn't about the grade range, but instead it is an educational philosophy. Middle schools usually have a team structure so there is more support and consistency in a student's day. A team (at least in our school) has common goals and regular meetings to support and care for kids). A junior high is structured more like a high school with more independence for students.
