yup. we provided a printout from our pediatrician with all the vaccines and dates he received them.

Yes, a yearly immunization record and contact info for her doctor.

in NY and CA, the daycares we went to were mandated by state law to have this info on file... this info, meaning a physician's physical exam report and vaccination record both dated within a year's time.

yup! we got a "physical form" from the pediatrician that included all the immunizations and everything.

Yes, they want a printed form and I have yet to get it (oooops) to them.

We have to give them her immunization records after her 6 month checkup; and then (I believe) update it yearly.

Yup. And I'm glad they do because I'm not comfortable with LO being with an unvaccinated child.

I have to provide an updated record to day care after every single vaccination. And if they haven't received it within a week or so, they ask me about it.

Yes, we have to provide an updated report after every vaccination. They keep track of which vaccines each kid needs and chase after you if you're behind on any.

We send our son to a home-based daycare and we also had to provide one.

yes we have only been there a few months but we did have to provide signed list from the doctor and since then we have to provide updates

We have to provide a health form with vaccine info when she started, and at 6 months, 12 months, 18 months, and 2 years. Then annually after that.
