DH hasn't been very good about packing the diaper bag. One time when he was with baby, we arranged to meet after work. When I asked him for something from the diaper bag, he handed me a bottle and nipple in her bottle bag, didn't bring diapers, wipes, spit-up cloth or toy. He's gotten a lot better since he spends about 3-4 hours with LO before I am off work but we still have some interesting outings if I don't double check things.
Is your SO good about packing the diaper bag?

Not at all! Haha we went out to dinner last night, no burp rag, no bottle, and only one diaper. It was a diaster! I'm going to make a checklist to have in her room from now on.

He's pretty good about packing the bag if I ask him to. He includes all that's needed. His problem, however, is just insisting that he won't be gone long enough to need one. To be fair, he's never taken the 8 month old anywhere alone. The 2 year old doesn't require nearly as much stuff.

I think the only time he's packed it I was narrating what to put in it. Usually if we're going on I pack the bag and he gets E dressed. He takes him out jogging sometimes and I don't think he even brings the bag! I think he'd probably remember to put diapers in it and we keep a travel case of wipes in there so he wouldn't have to remember those. We don't usually bring a bottle out with us but I doubt he would remember that, or toys.

We maintain separate diaper bags so he is in charge of keeps his stocked!

Not at all! He'll just throw everything in it & not have everything needed! He's tried packing it for me a couple times but I always end up re-packing it.

The bag always stays pretty stocked, but he know to check for an extra diaper and snack.

That's cute, DH is the same. 
I usually have a pre-packed diaper bag (minus milk) for him if he ever needs it.

Dh hasn't had to pack it by himself yet, and he's only packed it 2-3 times in 7 months. And he always asks me whats needed in there so it's usually just easier for me to pack it.

He helps me pack it on occasion, but I have to go in behind him and straighten it up - he has no idea where anything goes!

surprisingly, yes! he never takes her out, but once in awhile I've asked him to throw some things in for me and he does well.
