We usually ask our kids. We figure going out to eat is a treat and when they eat something they love everyone is usually happy.

We order for A because she's 15m. For D, we give him a limited list of the options on the menu and let him choose for himself. For example, last week we went to Applebee's and the menu options for him were mac and cheese, quesadilla, chicken fingers, grilled chicken tenders, corn dogs, and 2 cheeseburger sliders. I let him choose between the pizza, quesadilla, chicken fingers, and sliders. Because I'm not paying $4.50 for Kraft mac and cheese or Elios pizza, corn dogs are gross, and I know he wouldn't eat the grilled chicken tenders. So he has a choice, but not full reign of the menu.

DD is 2.5. We typically give her four or five options and let her pick. Sometimes all from the kids menu, sometimes from the regular menu, but most often a mix from both menus. We only eat out or get take out once a week at most so we are fine if she eats something less than healthy - we just want her to enjoy it!

She gets something off the kids meal and we let her choose which item she wants. Sometimes I'll eliminate an option so she doesn't choose it (aka: chicken fingers).

We give her choices based on what we think she'll eat. She doesn't eat that much at any given time so we don't stress about it being the healthiest, though we also won't let her only eat French fries

We give a couple choices depending on what we think he'll like. He's a pretty adventurous eater, though, so he usually doesn't order off the kids menu. We definitely don't just go for healthy food, though - one of his favorites is pancakes, so if pancakes are on the menu, we'll generally let him order that.

We give a couple of choices, instead of reading the whole menu--but we pick the choices based on what my kids will actually eat. And it's generally off the adult menu, but my kids will split it.
But they are more likely to eat salmon and broccoli than say, chicken fingers and mashed potatoes. Neither one of them really likes breaded chicken much and they don't like mashed potatoes.

We haven't eaten out much since DD2 was born; so before DD1 was 2, we ordered for her knowing what she likes.
Sometimes we get McDonalds and we ask what she wants but we know she always wants the same thing; burgers & fries haha.

Pretty much, although their options are usually pretty limited because of food allergies.
