My favorite tops to wear are the Liz Lange maternity tank tops from Target. They are comfortable, cheap, I can wear a nice fitting nursing bra, throw on a blazer or cardigan depending on the formality of where I'm going, and they're stretchy BUT don't stretch out so I can easily pull them down to nurse but not end up with a saggy neckline.
I still haven't gotten comfortable with the "pull up outer shirt/pull down under shirt", probably b/c my undershirt always wants to ride up when I go to pull the outer shirt up, so these work best for me.

I use Target nursing tanks a lot. Glamourmom ones too (better quality and I like the ones longer in length).

@runsyellowlites: I love those tanks for nursing!
I do the same when going out. I throw on a nursing bra with a tank over it. Then put on a button up shirt or cardigan over it. Usually with skinny jeans and boots.
I also found these crossover tees at target that make nursing when out easy.
At home I like comfy pants and a nursing cami. I like the gilligan o'malley ones from target. Or I wear a sleep nursing bra with a tank or tee over it.

I wore a Bravado nursing tank under everything for at least 8 months straight. It was supportive enough that I didn't need to wear a bra and slim/stretchy enough that I could put it under anything and everything.

@rachiecakes: I did too! I tried nursing wear from Target but I felt like I was flopping around!

I got some gap camis. They have good support. WEaring one now! It's spandexy and comfy.

@chopsuey119: I didn't wear them that often for nursing but I did LOVE gap maternity/nursing tanks & camis. So long & soft & stretchy. I still wear my maternity ribbed tanks to bed!
