Unsweetened cocoa powder (e.g., Ghiradelli's) + sugar, add just enough hot water to make it easier to mix, then fill up the mug with milk, stir, and microwave.

I boil water to pour over the powder. along time ago I started pre-mixing the powder with the milk to make sure everything mixes in.

I do both depending on my mood! Sometimes milk doesn't sit well in my stomach.
And sometimes I'll pour swiss miss into my coffee for an instant mocha!
