We just paid a $25 copay for every visit for IUI and monitoring.

@swedishfish: So, here is what's weird when I read the policy.. it says "infertility $25 per visit." There is no detail on procedures except IVF.

I'm looking up my EOBs now. We paid 10% of the contracted rate, though these are the OOP prices:

I spent about $300 in meds (out of pocket) and paid around $150 for all of my visits/bloodwork/etc. Insurance covered the rest.
Without coverage, we would have spent about $2500 for three cycles of TI.

@Mrs. Jump Rope: All of the tests are covered 100% and RE visits are $25 per co-pay. I feel like I need to talk to the RE billing department again, because I find it very hard to believe 0% of IUI is covered.

@littlek: I would talk to your insurance, not your REs billing department -- they're the ones you'll want to talk to if you have a question about an existing bill.
Do they have an insurance claims department? My REs office did, and they helped me navigate the IUI coverage and process (We were supposed to move on to IUI in January).

@Mrs. Jump Rope: My guess is I will try clomid first, to help me "ovulate stronger." At least, that is currently the direction the RE is planning on taking. I've got time to figure out the IUI, it's just baffling. My DH is also in the process of interviewing, so hopefully either (1) we are pregnant w/o needing IUI or (2) DH's new job has better benefits

@littlek: My 10 day supply of clomid (100mg) was $18 out of pocket. I got it filled at my regular pharmacy.
I have a vial of novarel sitting around that I ended up not needing, but had bought because they thought I would. I don't know what to do with it, but that's $120 down the drain!

@littlek: You should definitely talk to your insurance company again. Our plan on paper makes it sound like nothing is covered which worried us, and actually our first clinic billed us a $40 specialist copay for each monitoring visit because no one knew what was going on. Eventually they were forced to refund all of it because they realized that it was covered 100% under "testing" and only a consult required a copay.

@littlek: ask to meet with the RE's finance manager (or someone in that dept). I talked to someone at my insurance company and they gave me such wrong information. A quick 5 minute meeting with the finance person at the RE cleared it up, and way more was covered than the insurance company led me to believe. It may be that the "low tech" stuff isn't specifically mentioned but is still covered. My Infertility coverage policy (which I read top to bottom and still didn't understand) made it sound like 6 cycles of ART were covered, but what wasn't clear is that IUIs (and FETs) aren't considered ART, only fresh IVF. That didn't mean that IUIs and FETs aren't covered, just that they're not counted toward the lifetime max of 6 cycles. Good luck!

Here is what I got from the insurance webpage:
Infertility Services
Benefits for Infertility will be paid at the same level as Benefits for any other maternity-related procedure.
Diagnosis and treatment of infertility including but not limited to:
· Diagnosis and diagnostic tests.
· Medications.
· Surgery.
· In vitro fertilization.
· Embryo transfer.
· Zygote intra fallopian transfer.
· Intracytoplasmic sperm injection.
· Gamete intrafallopian transfer (GIFT).
· Four completed egg retrievals per lifetime of the Covered Person.
· Medical costs of egg and sperm donors.
Benefits for gamete intra fallopian transfer in vitro fertilization, and zygote intra fallopian transfer are limited to a Covered Person who meets all of the following conditions:
· The Covered Person has used all reasonable, less expensive and medically appropriate treatments and is still unable to become pregnant or carry a pregnancy.
· The Covered Person has not reached the limit of four completed egg retrievals.
· The Covered Person is 45 years of age or younger.
Services must be performed at facilities that conform to standards established by the American Society for Reproductive Medicine or the American College of Obstetrics and Gynecologists.
Limited to four completed egg retrievals per lifetime of the Covered Person.
Network
Depending upon where the Covered Health Service is provided, Benefits will be the same as those stated under each Covered Health Service category
The following infertility treatment-related services are not covered:
· Cryo-preservation and other forms of preservation of reproductive materials.
· Long-term storage of reproductive materials such as sperm, eggs, embryos, ovarian tissue, and testicular tissue.

@littlek: I would call for clarification on the IUI. Your coverage lists specific procedures that are not covered, but IUI isn't listed.. Though it's not listed under covered, either!
So frustrating. I think we should be as to read the materials from our insurance company and not have any questions

@littlek: I think they consider ART at the level of IVF/FET totally different from IUI. I can't find it in my policy right now but the language in my policy only addressed that injectibles/IVF/GIFT/ZIFT were not covered = infertility not covered at all. So we didn't realize for a long time that actually all testing, monitoring, and IUI were totally covered. It's likely that what you have is great coverage and you have everything covered through 4 IVF cycles.

@littlek: that's why I'd call. My IF rider says they cover diagnostic testing, but doesn't specify that it's covered AFTER you meet the deductible.
I cried when I got hit with a massive bill for my HSG. It was so much money & I didn't have the cash to cover it, so we went on a payment plan... And it delayed my treatments.

I can't remember exactly, but our one IUI was about $600 out of pocket I think. I agree with the others that you should call the insurance company and found out specifically what your IUI costs would be.

Out of pocket. No insurance coverage for me, other than inital IF testing.
$150 for ultrasound (sometimes I don't get charged this. I don't know what it is. Not going to complain when I don't get charged. )
$80 for trigger shot
$10 for femera pills (insurance covers it)
$150 for IUI
$200 to process DH's deposit for IUI
$250 to process DH's deposit for freezing.
Total: $690 per cycle.
I get Day 3 bloodwork every 6 months and I believe that might be covered under insurance.
If I wanted additional ultrasounds or bloodwork, I would have to pay $150 each.

My insurance paid for 50% of the IUI procedure but did not cover any of the drugs. I had to pay my co-pay for each of the monitoring appointments. It came out to be about $500 per cycle.

Monitored IUI+Clomid cycles were $550 I think, but I voted 0-500...

Assuming tomorrow is my last monitoring u/s and I get the green light for IUI Friday, this cycle will be:
Ultrasounds: $250 per ultrasound x 4
Meds: $225 per dose of gonal-f x 8
$145 per dose of ganirelix x 4
$70 ovidrel trigger x 1
Day of IUI: $480 for bloodwork, sperm wash and IUI procedure
= $3930 out of pocket. But then, I didn't respond well to last cycle on lower doses of meds, so that's really where my cost are this time around, the higher doses

So far I've paid $40 copay for my initial visit with RE (specialist). The monitor sessions haven't been billed, which I did 2x, so I dont think they charge for monitoring. (Or I'm being hopeful and they are just really slow). Blood work is typically about $30 after insurance. Ovidrel was $17. Progesterone was $112. We timed this cycle, so not sure about IUI charge yet. We have United Choice.

@bluestriped bee: that's way cheaper than the $1,600 per cycle we were told. But I looked around and that appears to be the going rate here.

$1600 "global fee" per cycle out of pocket which covered the IUI plus as many monitoring ultrasounds and bloodwork needed. Plus the $ for the meds. So happy for all you ladies who have infertility coverage!
