I didn't do a written birth plan. I had 2 things I knew I wanted- epidural and skin on skin right after she was born. I told the nurse these things multiple times.

I altered/personalized one that my doula uses as an example. It was pretty general. I think either way, the most important thing is that your preferences are discussed with your doctor(s) and the nurses are made aware of it. A well thought out birth plan is useless if both parties aren't at least open to discussing things.

I think I pulled a template off BabyCenter and copied/pasted the relevant items into a word doc to make my own. It ended up 1 page after I deleted the things I didn't care about/didn't apply.

@Goldilocks1107: yeah i think I will do something similar to you. Because the templates I've found all seem very long. I'm gonna create my own but use the templates as a guide

I looked up a few templates to get an idea of what I wanted to include, but then just composed my own.
Be sure to include an "in case of emergency/c-section". I'm glad I did. I had an in case of transfer and in case of c-section. I was supposed to deliver at a birth center bit developed pre-e which made me have to transfer so I'm soo glad i had that section!

My hospital has one so I plan on filling out that one thinking they'll be more receptive that way.

I used a bullet pointed list I created myself. I made it after doing a lot of reading on childbirth and pretty much copied down a few points from the books.

I used a template the hospital gave us during childbirth class, but the plan never made it out of my bag. Things went too fast for me to even think about it.
