What kind of information do you leave for a babysitter? I'm thinking our address, our phone numbers and a few other contact phone numbers. Then some brief notes about bedtimes but nothing over the top, since we aren't that picky about things. What else?

Definitely pediatrician number, number and address of pediatric urgent care. Medicine dosage. Number of our veterinarian.
We will be using a non family babysitter soon as well, so this is helpful!

lunch/Dinner optionsI'm typically never not available by phone, so I don't leave the doctor number because even if you call our doctor's office, you speak with a triage nurse and they basically tell you to come in for everything. It's not super helpful. And our doctor only works three days of the week, and not typically at night when you'd have a sitter. DH and I always keep our phones on us, and charged.

Probably address is the biggest thing, especially if it's just a sitter who is doing care for a couple hours or even a workday. That way if something emergent happens and they have to call 911, they can have the address available. Other than that, we aren't usually that far away, so I would expect the sitter to contact me if it's just a question/something urgent. I always have my phone out and available (even if at yoga or a doc appt or whatever).

I use that form too but also add our wifi information so our sitter can log on while we are out.
I also type up LO's current schedule/routine and where to find some of his things.
