It seems that I have overactive letdown, as one of my twins continues to choke/cough while nursing and the other just unlatches until the flow becomes manageable.
I've read about block feeding, but I don't want to reduce my supply - I need to feed two!
What else can I do to keep my little guy from choking during every nursing session? It scares the crap out of me and can't be very pleasant for him!

Have you tried pumping just a little at the beginning? This might make it easier for them to start, plus will keep the supply up.

This is coming from my breastfeeding class handout:
Lie on your side or lean back after the baby latches to slow the flow of milk
Position the baby in a more upright or sitting position to help the baby cope with the fast flow
Express some milk by hand or pump to decrease pressure and slow the flow of milk

Sounds like the one twin got a good idea, and you might have to teach the other twin by unlatching him and catching the worst of it in a cloth. My guy got really good at pretty much spitting it out for the first few minutes....it was always such a mess.
I have had some success with feeding upright / nursing 'uphill' (google for some pics).
Prepare to be covered in milk for the next month or two or three until they get big enough to handle the flow! (They seem to become champion chuggers after a while.)
And now I bow down to you, mother of twins....I am amazed just thinking about what that would be like.

But I did want to say you are super woman! I can't even imagine having twins and you make it seem easy! Hats off to you and your awesome mommyness

Yeah, I would either pump a bit off first, or let it spray off into a cloth until it slows a bit. And as was mentioned, if you can lay back a bit and have them more on top, it helps.
