My BFF has been trying to get pregnant for about 1.5 years, and this cycle she was supposed to start injectibles with IUI. She did the injectables, and today when she went in she had 5 follicles, 1 at 20 and 4 at 16. Her doctor told her that he wouldn't cancel it, and it was up to her if she wanted to do the IUI or cancel and try again next month with a lower dosage of meds. He said that there was about a 10% chance that all five would ovulate.
She wanted my opinion for what to do, and I basically just validated what she chose to do (cancel this cycle due to risk of high level multiples).
What would you do?

Personally, I'd probably still go ahead with the cycle.
In Ontario, that would be an automatically cancelled cycle.

@LAZB: That would be an automatic cancellation at our clinic (although they can't stop what happens at home). For me I wouldn't go through with it because the risk of very high order multiples will be high, such a crappy situation.
ETA: Did they not monitor her during the stims? They should have known before today that there were that many follicles. She should push for more monitoring during her next cycle

That would be an automatic cancel at my REs office. They canceled if you had more than two follicles.
Was she not monitored?

They did monitor, and yesterday they told her that all five were the same size, so they would probably cancel. I'm not sure why the doctor changed his mind today.

I would cancel. Not worth the risk! My RE wouldn't have allowed it. Over 3 and automatic cancellation.

@LAZB: Well it seems weird that they didn't scale back her stims earlier in the cycle... she should never have got to 5 follicles that size.

I would also cancel. I don't know how I could make the decision of reduction if it came to that.

Remember Jon & Kate + 8? I believe they were in this same situation with their sextuplets. I think it would come down to how your friend feels about selective reduction. If she is OK with it, then go forward with the IUI or trying on their own. If not, then hold off for sure.

Cancel. That's because I highly respect my RE, and like @Jenn23:'s RE, they cancel at 3.....although they did say last cycle that they might consider going ahead if I got to 4, but we would have the serious discussion about multiples.
It's hard skipping, especially with the high cost of injectables. But I think it's better to play it safe. And look into finding a new RE--I wouldn't want to feel like I had to make those big decisions on my own.
ETA: this just reminded me that I had a dream last night my big sister was pregnant with 5 babies. Everyone was happy for her, and I was the only one asking what was going to happen. Ha!

@Mrs.Someone: she wouldn't want To do selective reduction, so that was a big argument against doin this cycle.

@LAZB: If I wasn't open to selective reduction I'd definitely cancel the cycle (although I'd most likely cancel it anyway)

i had a similar situation where i had 4 mature follicles. my RE was willing to go ahead with the cycle but gave me a very long talking to about the chances of multiples and was obviously relieved when i mentioned that selective reduction would be used if more than 3 implanted.
we went forward with the cycle, i ovulated 3 eggs and got pregnant with LO#2 who is due in 9.5 weeks. SO SO SO glad i went for it, but obviously that's just my situation. good luck to your friend!
eta: i should have read all the responses! i just saw that your friend would not be ok with selective reduction, so if it were me and i felt that way, i would cancel the cycle.

I would cancel...it's not worth the risk. Jon and Kate plus 8, anyone?
ETA - actually I think Jon and Kate's IUI cycle was cancelled and they got pregnant naturally with all of those follicles.

@LAZB: Then definitely cancel. OR maybe check to see if they are eligible for mini-IVF? Spaniellove could pass on some info on what it entails.

I would have cancelled but still BD at home. But, our issue was male factor, so even then it likely wouldn't have worked.

@Mrs. Pinata: I'm pretty certain that BD at home after a cancelled IUI is how Jon and Kate got all those babies!

@swedishfish: True! I guess with our track record I would have chanced it. And they also didn't have MF, so he had a bunch of good sperm to get to all those eggs. So, I think that's a big difference.

I would cancel since I don't think I could selectively reduce in that situation.

I like your idea of mini IVF if they were open to that. Otherwise I would cancel too I think. Too risky.
