I live in the NYC area, and my doctor has privileges at St. Luke's - Roosevelt, which has an in-hospital birthing center in addition to a regular L&D ward.

I want to try for a med free birth because I want to avoid the intervention spiral: epidural --> need for pitocin --> need for stronger epidural --> need for more pitocin --> stalled labor --> c-section. I don't know if I will be able to do it in the moment, but I want to try.

I am only 7w pregnant, so it's early, but I am trying to do a lot of research in advance to figure out what kind of strategies I should employ for this process. I also want to make sure that my medical provider is aligned with my preferences so that I have time to switch providers if necessary. It sounds like, if you don't educate/prepare yourself and your partner, then a fully medicated hospital birth with high probability of c-section will probably happen *to* you.

So far I am considering the following:

1. Hiring a doula to help my husband advocate for me at the hospital.

2. Educating my husband, my doula and my doctor regarding my birth preferences.

3. Consider using a midwife instead of an obstetrician -- this is probably not going to happen since I like my doctor, but it's not entirely off the table.

4. Consider a home birth -- this is almost certainly off the table because if something *did* go wrong and require emergency medical intervention, I would never forgive myself.

5. Delay going to the hospital as long as possible once labor sets in. Apparently a lot of doctors simply get tired of waiting and start pushing for more intervention just to speed things along. Waiting until it's absolutely necessary to go to the hospital will increase the amount of time you have to labor before intervention.

6. Accept that I can't control the entire process and prepare myself to take things as they come.

Am I overlooking anything?
