For me lately, it's Mail, iMessages, Safari and Instagram. Instapaper and Kindle too, and oh yah - the Phone app!
I don't take a ton of pictures, but the Camera app gets a fair amount of use on weekends. Plus I always seem to be in the Settings app, to increase or lower the brightness of my phone.

I use FB, IG, Goodreads, Pandora, Weather and I know this isn't an app but I'm always clicking on my HB icon, as well as setting alarm clocks and using the calculator.

FB, IG, VSCO, camera, Songza, WWF, Google Chrome, Foscam, Baby Connect, and Reddit. They all get used daily, or nearly.

@mrbee: The HB one, of course!! Oh wait...there isn't one (yet?!)...but if there was THAT would get the most use!
Safari, the camera app, Baby Connect. DS deleted my other apps like FB, Pinterest and Etsy, so I need to update and reinstall them, but I've been too lazy to do so!
