Xander's 3.5 months and my diaper bag has:

Couple outfit changes
Diapers (~5)
Wipes
Burp Cloths
Blankets
Pacifier

Need to add bibs and also change out the outfits since he has probably outgrown them. Also, when I actually go out with him and the diaper bag, I throw in my wallet, phone, and water bottle.

At 2 years:
- change of underpants and shorts
- wipes and boogie wipes
- hat
- sippy and snack

LO is 5.5 months

Diaper bag is a big old jcrew satchel.

For baby:
JJ Cole diaper clutch (changing pad, 5 diapers, wipes case)
Spare outfit
Spare socks
Sweater
Baby Einstein take along tunes
A couple pacifiers
Face wipes
A&A blanket
Empty gallon sized freezer zip lock bag
Sun hat

For dog:
Dog poop bags
zip lock of liver treats

For me:
Spare hair tie
Wallet
Burt's bees lip balm
Cell phone
Hand cream
Hand sanitizer
Spare socks

I am a huge diaper bag minimalist SO for my 14 mth old I have:

2 diapers w/ liners
wipes
wetbag
changing mat/sometimes a blanket instead
sometimes a change of clothes (if I remember to put them in there)
sometimes snacks (only if we'll be gone awhile)
her cup... only when we go to church (usually I'd just nurse her)
My wallet & my keys

R is 3.5 months
-5 or 6 Diapers
-Wipes
-Hand sanitizer
-Diaper cream
-2 extra bibs
-2 burp cloths
-changing pad (came with bag)
-changing pad disposable liners
-outfit change
-A&A blanket
-Nursing cover
-Ktan
-Disposable bags for diapers
-Bottle with water
-Formula (just in case)
- 6 pacifiers (that she never takes, I need to take them out)
- usually a headband

It's changes depending on where we're going! A lot
Less stuff for a toddler: 16 months

2-3 diapers, wipes
Sunscreen
Change of clothes
Pacifier
Toy
Snack

And if we're going out to eat, a bib
